Ptyalin

The name of the ENZYME contained in the SALIVA, by which starchy materials are changed into sugar, and so prepared for absorption. It is identical to the AMYLASE of pancreatic juice. (See DIGESTION; PANCREAS.)... ptyalin

Saliva

The slightly alkaline fluid that is secreted into the mouth by the salivary glands and the mucous membranes lining the mouth. Saliva contains the enzyme amylase, which helps to break

down carbohydrates (see digestive system). Saliva keeps the mouth moist, lubricates food to aid swallowing, and facilitates the sense of taste.... saliva

Agaricus Campestris

Linn.

Synonym: Psalliota campestris (Linn.) Fr.

Family: Agaricaceae.

Habitat: The fungi is distributed in many parts of India, particularly on the hills and plains of northern and eastern India. Grows during the rainy weather on dead organic matter, e.g. rotting leaves and manure.

English: Field mushroom, Edible mushroom.

Ayurvedic: Chhatraka, Bhuumi- chhatra.

Unani: Kammat.

Siddha: Venkodiveli.

Folk: Khumbi.

Action: A protein (2.74%) supplement and an excellent source of vitamins of B complex. Vitamins K, C and D are also present. Though all the amino acids are reported to be present, the concentration of tryptophane is particularly low.

Extracts of A. campestris contain tyrosinase; lowered blood pressure of hypertensive animals when administered intravenously (exhibited no effect on normal animals).

Fungal enzyme preparations have been used in digestive diseases.

Amorphophallus Campanulatus

(Roxb.) Blume ex Decne.

Family: Araceae.

Habitat: Native to tropical Asia; cultivated throughout India.

English: Elephant-foot Yam.

Ayurvedic: Suurana, Kanduula, Arshoghna, Kand-ayak, Kandala.

Unani: Zamin-qand, Zamikand.

Siddha/Tamil: Chenaikkizhangu. Kaathukarunai (wild var.)

Action: Corm is prescribed in bronchitis, asthma, abdominal pain, emesis, dysentery, enlargement of spleen, piles, elephantiasis, diseases due to vitiated blood, rheumatic swellings.

Along with other therapeutic applications, The Ayurvedic Pharmacopoeia of India indicates the use of corm in prostatic hyperplasia.

(The corm is irritant due to the presence of calcium oxalate. It can be con- sumedafterit iswashedwell andboiled in tamarind water or butter milk.)

The corm contains an active di- astatic enzyme amylase, betulinic acid, tricontane, lupeol, stigmasterol, beta- sitosterol and its palmitate and glucose, galactose, rhamnose and xylose.

Intake of 3.6-7.2 g of konjac (Amor- phophallus konjac tuber) mannan for 90 days reduced the dose of insulin or hypoglycaemic drugs (in human clinical study). (Francis Brinker.)

About the Nutrients in This Food A banana begins life with more starch than sugar, but as the fruit ripens its starches turn to sugar, which is why ripe bananas taste so much better than unripe ones.* The color of a banana’s skin is a fair guide to its starch/ sugar ratio. When the skin is yellow-green, 40 percent of its carbohydrates are starch; when the skin is fully yellow and the banana is ripe, only 8 per- cent of the carbohydrates are still starch. The rest (91 percent) have broken down into sugars—glucose, fructose, sucrose, the most plentiful sugar in the fruit. Its high sugar content makes the banana, in its self-contained packet, a handy energy source. Bananas are a high-fiber food with insoluble cellulose and lignin in the tiny seeds and soluble pectins in the flesh. They are also a good source of vitamin C and potassium. One small (six-inch) banana or a half-cup of sliced banana has 2.6 g dietary fiber and 8.8 mg vitamin C (12 percent of the R DA for a woman, 10 percent of the R DA for a man), plus 363 mg potassium.

The Most Nutritious Way to Serve This Food Fresh and ripe. Green bananas contain antinutrients, proteins that inhibit the actions of amylase, an enzyme that makes it possible for us to digest * They are also more healt hful. Raw bananas are richer in potassium than cooked bananas; heating depletes potassium.

Buying This Food Look for: Bananas that will be good when you plan to eat them. Bananas with brown specks on the skin are ripe enough to eat immediately. Bananas with creamy yellow skin will be ready in a day or two. Bananas with mostly yellow skin and a touch of green at either end can be ripened at home and used in two or three days. Avoid: Overripe bananas whose skin has turned brown or split open. A grayish yellow skin means that the fruit has been damaged by cold storage. Bananas with soft spots under the skin may be rotten.

Storing This Food Store bananas that aren’t fully ripe at room temperature for a day or two. Like avocados, bananas are picked green, shipped hard to protect them from damage en route and then sprayed with ethylene gas to ripen them quickly. Untreated bananas release ethylene natu- rally to ripen the fruit and turn its starches to sugar, but natural ripening takes time. Artificial ripening happens so quickly that there is no time for the starches to turn into sugar. The bananas look ripe but they may taste bland and starchy. A few days at room temperature will give the starches a chance to change into sugars. Store ripe bananas in the refrigerator. The cold air will slow (but not stop) the natural enzyme action that ripens and eventually rots the fruit if you leave it at room temperature. Cold storage will darken the banana’s skin, since the chill damages cells in the peel and releases polyphenoloxidase, an enzyme that converts phenols in the banana peel to dark brown compounds, but the fruit inside will remain pale and tasty for several days.

Preparing This Food Do not slice or peel bananas until you are ready to use them. When you cut into the fruit, you tear its cell walls, releasing polyphenoloxidase, an enzyme that hastens the oxidation of phenols in the banana, producing brown pigments that darken the fruit. (Chilling a banana produces the same reaction because the cold damages cells in the banana peel.) You can slow the browning (but not stop it completely) by dipping raw sliced or peeled bananas into a solution of lemon juice or vinegar and water or by mixing the slices with citrus fruits in a fruit salad. Overripe, discolored bananas can be used in baking, where the color doesn’t matter and their intense sweetness is an asset.

What Happens When You Cook This Food When bananas are broiled or fried, they are cooked so quickly that there is very little change in color or texture. Even so, they will probably taste sweeter and have a more intense aroma than uncooked bananas. Heat liberates the volatile molecules that make the fruit taste and smell good.

How Other Kinds of Processing Affect This Food Drying. Drying removes water and concentrates the nutrients and calories in bananas. Bananas may be treated with compounds such as sulfur dioxide to inhibit polyphenoloxi- dase and keep the bananas from browning as they dry. People who are sensitive to sulfites may suffer severe allergic reactions, including anaphylactic shock, if they eat these treated bananas. Freezing. Fresh bananas freeze well but will brown if you try to thaw them at room tem- perature. To protect the creamy color, thaw frozen bananas in the refrigerator and use as quickly as possible.

Medical Uses and/or Benefits Lower risk of stroke. Various nutrition studies have attested to the power of adequate potassium to keep blood pressure within safe levels. For example, in the 1990s, data from the long-running Harvard School of Public Health/Health Professionals Follow-Up Study of male doctors showed that a diet rich in high-potassium foods such as bananas, oranges, and plantain may reduce the risk of stroke. In the study, the men who ate the higher number of potassium-rich foods (an average of nine servings a day) had a risk of stroke 38 percent lower than that of men who consumed fewer than four servings a day. In 2008, a similar survey at the Queen’s Medical Center (Honolulu) showed a similar protective effect among men and women using diuretic drugs (medicines that increase urination and thus the loss of potassium). Improved mood. Bananas and plantains are both rich in serotonin, dopamine, and other natural mood-elevating neurotransmitters—natural chemicals that facilitate the transmis- sion of impulses along nerve cells. Potassium benefits. Because potassium is excreted in urine, potassium-rich foods are often recommended for people taking diuretics. In addition, a diet rich in potassium (from food) is associated with a lower risk of stroke. A 1998 Harvard School of Public Health analysis of data from the long-running Health Professionals Study shows 38 percent fewer strokes among men who ate nine servings of high potassium foods a day vs. those who ate less than four servings. Among men with high blood pressure, taking a daily 1,000 mg potas- sium supplement—about the amount of potassium in one banana—reduced the incidence of stroke by 60 percent.

Adverse Effects Associated with This Food Digestive Problems. Unripe bananas contain proteins that inhibit the actions of amylase, an enzyme required to digest starch and other complex carbohydrates. Sulfite allergies. See How other kinds of processing affect this food. Latex-fruit syndrome. Latex is a milky fluid obtained from the rubber tree and used to make medical and surgical products such as condoms and protective latex gloves, as well as rub- ber bands, balloons, and toys; elastic used in clothing; pacifiers and baby bottle-nipples; chewing gum; and various adhesives. Some of the proteins in latex are allergenic, known to cause reactions ranging from mild to potentially life-threatening. Some of the proteins found naturally in latex also occur naturally in foods from plants such as avocados, bananas, chestnuts, kiwi fruit, tomatoes, and food and diet sodas sweetened with aspartame. Persons sensitive to these foods are likely to be sensitive to latex as well. NOTE : The National Insti- tute of Health Sciences, in Japan, also lists the following foods as suspect: Almonds, apples, apricots, bamboo shoots, bell peppers, buckwheat, cantaloupe, carrots, celery, cherries, chestnuts, coconut, figs, grapefruit, lettuce, loquat, mangoes, mushrooms, mustard, nectar- ines, oranges, passion fruit, papaya, peaches, peanuts, peppermint, pineapples, potatoes, soybeans, strawberries, walnuts, and watermelon.

Food/Drug Interactions Monoamine oxidase (MAO) inhibitors. Monoamine oxidase inhibitors are drugs used to treat depression. They inactivate naturally occurring enzymes in your body that metabolize tyra- mine, a substance found in many fermented or aged foods. Tyramine constricts blood vessels and increases blood pressure. If you eat a food containing tyramine while you are taking an M AO inhibitor, you cannot effectively eliminate the tyramine from your body. The result may be a hypertensive crisis. There have been some reports in the past of such reactions in people who have eaten rotten bananas or bananas stewed with the peel. False-positive test for tumors. Carcinoid tumors—which may arise from tissues of the endo- crine system, the intestines, or the lungs—secrete serotonin, a natural chemical that makes blood vessels expand or contract. Because serotonin is excreted in urine, these tumors are diagnosed by measuring the levels of serotonin by-products in the urine. Bananas contain large amounts of serotonin; eating them in the three days before a test for an endocrine tumor might produce a false-positive result, suggesting that you have the tumor when in fact you don’t. (Other foods high in serotonin are avocados, eggplant, pineapple, plums, tomatoes, and walnuts.)... bananas

Citrullus Vulgaris

Family: Cucurbitaceae.

Habitat: Cultivated throughout India on sandy river beds, up to an altitude of 1,500 m.

English: Watermelon.

Ayurvedic: Kalinga.

Unani: Tarbuz.

Siddha: Poiychaviral, Tharbuza- palam (Tamil.

Action: Pulp—cooling and refreshing, a rich source of pectin, carotenoids, surcose (as major sugar). Fruit juice is prescribed in strangury and urinary complaints, also in hepatic congestion and intestinal catarrh. Seeds—cooling, purgative, diuretic, demulcent (used in urinary infections). Leaves— febrifuge. The pericarp is given in diarrhoea.

Watermelon juice contains citrullin (0.17%) and arginine, which are thought to increase urea production in the liver, thus increasing the flow of urine.

The seeds possess a high lipase activity comparable to that of wheat germ, in addition to high lipoxygenase, urease and trypsin-inhibitor activities. Aqueous extract of the seeds also exhibit amylase inhibitor activity. The seed oil is used as a substitute for almond oil.

The roots of mature plant contain a triterpene, bryonolic acid. Bryono- lic acid possesses a stronger antiallergic activity with lesser side effects than that of glycyrrhetinic acid, the aglycone of glycyrrhizin, used clinically in Japan for the treatment of allergy and hepatitis.... citrullus vulgaris

Pancreatin

Pancreatin preparations (often in the form of a powder) contain the four powerful enzymes (see ENZYME), trypsin, chymotrypsin, lipase, and amylase, which continue the digestion of foods started in the stomach (see PANCREAS – Functions; DIGESTION). They are given by mouth for the relief of pancreatic de?ciency in conditions such as pancreatitis (see PANCREAS, DISORDERS OF) and CYSTIC FIBROSIS. Pancreatin is also used for the preparation of pre-digested, or so-called peptonised, foods, such as milk and some starchy foods.... pancreatin

Parotid

A pair of salivary glands tucked into the notch in front of each ear and emptying through parotid ducts by each upper 2nd molar. Although the fluid has some of the thick viscous lubricant nature of saliva from the glands in the floor of the mouth, the parotids secrete high levels of ptyelin and amylase (starch-digesting enzymes) lysozymes (antimicrobial enzymes) and a group of proteins loosely called parotin that stimulate epithelial and nerve cell growth...a lot more here than just spit.... parotid

Ipomoea Batatas

(Linn.) Lam.

Habitat: Native to tropical America; cultivated throughout India for edible tubers.

English: Sweet potato.

Ayurvedic: Mukhaaluka, Rataalu, Raktaalu, Raktapindaka, Raktakan- da.

Siddha/Tamil: Sakkareivelleikulan- gu.

Unani: Shakarkand, Rataalu.

Action: Root—used in strangury, urinary discharges, burning sensation, thirst. Whole plant—used in low fever and skin diseases.

Cooked tubers contain reducing sugars 6.45, sucrose 2.23, maltose 864, dextrins 0.51 and polysaccharides 14-13%. Cooking increases the sweetness as a result of the hydrolysis of starch to maltose and dextrins through the action of beta-amylase.

Sweet potatoes are rich in starch content. During the storage a part of starch content is converted into reducing sugars and subsequently into sucrose. In a sample stored for 5 months, the starch content was reduced from 19.1% to 14.1% while the percentage of reducing sugars (as dextrose) and sucrose increased from 0.9 to 1.7 and 1.9 to 6.1% respectively.

Indian types with white flesh contain little or no carotene, while American types with pink flesh contain as high as 5.4-7.2 mg/100 g of carotene. Vitamins present in the tubers are : thiamine 0.09-0.14, riboflavin 0.05-0.10 and vitamin C 16-22 mg/100 g.

The hot aqueous extract of leaves exhibits significant inhibitory activity of rat lens aldose reductase (AR). Ellagic and 3,5-dicaffoylquinic acids have been isolated as potent inhibitors.

The leaves also contain polysaccha- rides which increase the platelet count in experimental animals due to enhanced production ofthrombopoietin.

From the stem and root, hexadecyl, octadecyl and eicosyl p-coumarates have been isolated.

The tubers show significant lectin activity and exhibit haemagglutinating activity in trypsinized rabbit erythro- cytes.... ipomoea batatas

Laurus Nobilis

Linn.

Family: Lauraceae.

Habitat: Native to the Mediterranean region; cultivated throughout India.

English: Laurel, Sweet Bay.

Unani: Habb-ul-Ghaar, Daphni.

Action: Leaves—mild sedative, gastric tonic, cholagogue, diaphoretic, antiseptic, antifungal. Used as a gargle against sore throat. Oil—used externally for rheumatism and in hair dressings for dandruff. Berry— emmenagogue, antileucorrhoeic, antidiarrhoeal.

Fresh leaves from Lahore (Pakistan) gave an essential oil (0.3-0.36%) with, 1,8-cineole 42.2, eugenol 16.4, sabinene 6.5, alpha terpineol 1.6, alpha-pinene 3.4, methyl eugenol 2.4 and terpinolene 1.9%. Major components of Greek and Russian oils were 1,8-cineole followed by alpha-terpinyl acetate.

The fruit from Kumaon region gave an essential oil (5%), including among others, 1,8-cineol (28.4), methyl cinna- mate (20.1), alpha phellandrene (10.1) and alpha-pinene (9.3%).

The leaves contain sesquiterpene lactones and isoquinoline alkaloids. Ethanolic extract of leaves produces a significant decrease in blood glucose level of diabetic rabbits. It contains amylase inhibitors which can supress sugar metabolism and can be used as an antiobesity agent for pet animals.

The leaf extract has been used as an antidandruff solution.... laurus nobilis

Pancreas

A gland situated in the back of the abdomen, at the level of the ?rst and second lumbar vertebrae. It lies behind the lower part of the stomach, an expanded portion – called the head of the pancreas – occupying the bend formed by the duodenum or ?rst part of the small intestine, whilst a long portion – known as the body

– extends to the left, ending in the tail which rests against the spleen. A duct runs through the whole gland from left to right, joined by many small branches in its course, and, leaving the head of the gland, unites with the bile duct from the liver to open into the side of the small intestine about 7·5–10 cm (3–4 inches) below the outlet of the stomach.

Scattered through the pancreas are collections of cells known as the ISLETS OF LANGERHANS, of which there are around a million in a normal individual. These do not communicate with the duct of the gland, and the internal secretion of the pancreas – INSULIN – is formed by these cells and absorbed directly into the blood.

Functions The most obvious function of the pancreas is the formation of the pancreatic juice, which is poured into the small intestine after the partially digested food has left the stomach. This is the most important of the digestive juices, is alkaline in reaction, and contains (in addition to various salts) four enzymes (see ENZYME) – TRYPSIN and CHYMOTRYPSIN, which digest proteins; AMYLASE, which converts starchy foods into the disaccharide maltose; and LIPASE, which breaks up fats. Inadequate production of insulin by the islets of Langerhans leads to the condition known as DIABETES MELLITUS. In addition to insulin, another hormone is produced by the pancreas: this is glucagon which has the opposite e?ect to insulin and raises the blood sugar by promoting the breakdown of liver glycogen.... pancreas

Dextrin

n. a carbohydrate formed as an intermediate product in the digestion of starch by the enzyme amylase. Dextrin is used in the preparation of pharmaceutical products (as an *excipient) and surgical dressings.... dextrin

Pancreatic Enzyme Replacement Therapy

(PERT) a therapeutic approach using supplements of the digestive enzymes lipase, amylase, and protease (Creon, Pancrex), which are given when there is insufficient endogenous production of pancreatic enzymes: for example, to patients with pancreatic cancer or cystic fibrosis. Pancreas, Disorders Of

Pancreatic cancer The incidence of pancreatic cancer is rising: around 7,000 cases are now diagnosed annually in the UK, accounting for 1–2 per cent of all malignancies. There is an established association with heavy cigarette-smoking, and the cancer is twice as common in patients with diabetes mellitus as compared with the general population. Cancer of the pancreas is hard to diagnose; by the time symptoms occur the tumour may be di?cult to treat surgically – with PALLIATIVE bypass surgery the only procedure.

Chronic pancreatitis may be painless; it leads to pancreatic failure causing MALABSORPTION SYNDROME and diabetes mellitus, and the pancreas becomes calci?ed with shadowing on X-RAYS. The malabsorption is treated by a low-fat diet with pancreatic enzyme supplements; the diabetes with insulin; and pain is treated appropriately. Surgery may be required.

Acute pancreatitis An uncommon disease of the pancreas which may start gradually or suddenly, usually accompanied by severe abdominal pain which often radiates through to the back. Biliary tract disease and alcohol account for 80 per cent of patients admitted with acute pancreatitis, while other causes include drugs (see AZATHIOPRINE and DIURETICS) and infections such as MUMPS. Patients are acutely ill with TACHYCARDIA, fever and low blood pressure; many go into SHOCK. The condition may be mistaken for a perforated PEPTIC ULCER, except that in acute pancreatitis the blood concentration of AMYLASE is raised. The main complication is the formation of a PSEUDOCYST. Treatment includes intravenous feeding, ANTICHOLINERGIC drugs and ANALGESICS. Regular measurements of blood GLUCOSE, CALCIUM, amylase and blood gases are required. Abdominal ULTRASOUND may identify gall-stones (see under GALL-BLADDER, DISEASES OF). If the patient deteriorates, he or she should be admitted for intensive care as haemorrhagic pancreatic necrosis may be developing. LAPAROTOMY and DEBRIDEMENT may be called for. Raphanus Sativus

Linn.

Family: Cruciferae; Brassicaceae.

Habitat: Cultivated in Uttar Pradesh, Punjab, Maharashtra and Gujarat.

English: Radish.

Ayurvedic: Muulaka, Laghu- muulaka, Muulakapotikaa, Visra, Shaaleya, Marusambhava. Pods— Sungraa, Singri, Mungraa.

Unani: Muuli, Turb Fajal.

Siddha/Tamil: Mullangi.

Action: Radish—preparations are used in liver, gallbladder and urinary complaints. Green leaves— diuretic and carminative. Seeds— diuretic, purgative, expectorant.

A decoction of dry radish is given orally in piles. Extract of the dry root is given for hiccough, influenza, dysentery, colic and urinary troubles.

Key application: In peptic disorders, especially those related to dyskinesia of the bile ducts; and in catarrhs of the upper respiratory tract. (German Commission E.)

The Ayurvedic Pharmacopoeia of India recommends the juice of the whole plant in sinusitis; juice of the root in diseases of the throat and sinusitis; and the seed in amenorrhoea, cough and dyspnoea.

The fleshy root and seeds contain trans-4-methyl-thiobutenyl isothio- cyanate glucoside (the pungent principle), cyanidin-5-glucoside-3-sophoro- side, pelargonidin diglycoside, cyani- din diglycoside, 5-methyl-L-cysteine- sulphoxide (methiin), steroidal sa- pogenins and sulphorophene.

The enzymes present in the radish are phosphatase, catalase, sucrase, amylase, alcohol dehydrogenase and pyruvic carboxylase.

Radish contains caffeic acid and fer- ulic acid which exhibit hepatoprotec- tive and choleretic properties. It contains choline which prevents deposition of fat in liver. Amino acids, or- nithine, citrulline, arginine, glutamic acid and asparatic acid remove toxins from the body and urea acumulation.

Radish is a good source of ascorbic acid (15-40 mg/100 g), trace elements include aluminium, barium, lithium, manganese, silicon, titanium, also iodine (upto 18 mcg/100 g) and ascor- bigen.

Roots, leaves, flowers and pods are active against Gram-positive bacteria.

The seeds are reported to contain a broad spectrum antibiotic, machro- lysin, specific against Mycobacterium tuberculosis. Raphanin, extracted from the seeds, is active against Grampositive and Gram-negative bacteria.

A purified basic protein, homologous to nonspecific lipid transfer proteins, from seeds showed antifungal activity.

Raphanus caudatus Linn., synonym R. sativus var. caudatus, is known as Rat-Tail Radish.

A native to Java, it is cultivated in northern and western India. The root is not used; pods, purple or violet in colour, are consumed for properties attributed to Raphanus sp. These are known as Mungraa or Sungraa.

Dosage: Whole plant-20-40 ml juice; root—15-30 ml juice. (API, Vol. II.) Seed—1-3 g powder. (API, Vol. III.)... raphanus sativus

Pancreatic Juice

the digestive juice secreted by the *pancreas. Its production is stimulated by hormones secreted by the duodenum, which in turn is stimulated by contact with food from the stomach. If the duodenum produces the hormone *secretin the pancreatic juice contains a large amount of sodium bicarbonate, which neutralizes the acidity of the stomach contents. Another hormone (see cholecystokinin) stimulates the production of a juice rich in digestive enzymes, including trypsinogen and chymotrypsinogen (which are converted to *trypsin and *chymotrypsin in the duodenum), *amylase, *lipase, and *maltase.... pancreatic juice

Starch

n. the form in which *carbohydrates are stored in many plants and a major constituent of the diet. Starch consists of linked glucose units and occurs in two forms, ?-amylose and amylopectin. In ?-amylose the units are in the form of a long unbranched chain; in amylopectin they form a branched chain. The presence of starch can be detected using iodine: ?-amylose gives a blue colour with iodine; amylopectin a red colour. Starch is digested by means of the enzyme *amylase. Saccharum Officinarum

Linn.

English: Sugarcane, Noble Cane.

Ayurvedic: Ikshu, Dirgha-chhada, Bhuurirasa, Morata, Asipatra, Madhutrna, Gudamuula, Trnarasa.

Unani: Gannaa, Naishakar.

Siddha/Tamil: Karumbu, Nanal.

Action: Cane Juice—restorative, cooling, laxative, demulcent, diuretic, antiseptic. Used in general debility, haemophilic conditions, jaundice and urinary diseases.

The Ayurvedic Pharmacopoeia ofIn- dia recommends the juice of the stem in haemorrhagic diseases and anuria; and the root in dysuria.

Sugarcane juice contains surcose (70-80% of soluble solids in the juice), glucose and fructose. Non-sugar constituents present in the cane juice are carbohydrates other than sugars. As- paragine and glutamine are prominent amino acids in the juice. Other amino acids include alanine, gamma- amino butyric acid, aspartic and glutamic acids, glycine, leucine, lysine, serine and tyrosine. The presence of phenylalanine, histidine, valine, proline, threonine and arginine, pipecolic acid, methionine and tryptophan has also been reported.

Aconitic acid constitutes about three-fourths of the total carboxylic acid present in the juice.

Vitamins present in the juice are: thiamine, riboflavin, niacin, pantothenic acid, biotin, and vitamin D; enzymes include diastase, invertase, lac- tase, peroxidase, tyrosinase.

Phenols in the cane juice are mainly polyphenols from tannin and antho- cyanin from the rind.

Cane juice contains glycolic acid which improves skin complexion as it has antiwrinkle effect, prevents scaly growth and increases natural collagen and elastin in the skin.

Enzymes present in the seeds include large quantities of diastase and invertase.

An ester, vanilloyl-l-O-beta-D-glu- coside, has been isolated from the bagasse.

The leaves contain alpha-amylase and glutathione-S-transferase.

Dosage: Stem—200-400 ml juice; rootstock—15-30 g for decoction. (API, Vol. IV.)... saccharum officinarum

Scirpus Kysoor

Roxb.

Synonym: S. grossus Linn. f.

Family: Cyperaceae.

Habitat: Distributed throughout India, especially in swamps, up to an altitude of 700 m.

Ayurvedic: Kasheru, Kasheruka.

Siddha/Tamil: Karundan, Gundati- gagaddi (rhizome).

Folk: Kaseru.

Action: Tuber—nutritious, astringent, antidiarrhoeal, antiemetic, galactagogue, hypoglycaemic, diuretic, urinary antiseptic. Used in prescriptions for dysuria, diabetes, genitourinary affections, dyscrasia and as a spermopoietic and liver tonic.

The Ayurvedic Pharmacopoeia ofln- dia recommends the powder of the rhizome for promoting spermatogen- sis and development of breast.

The tuber gave progesterone, sugars, tannins, starch and saponins. The fruit contains amylase.

The tuber of Scirpus lacustris L. (Kashmir, Ladakh, Kumaon), known as Great Bulrush or Clubrush, is also used as astringent, diuretic and antimicrobial. The aromatic compounds isolated from the rhizomes include derivatives of benzaldehyde, hydrox- ybenzoic and cinnamic acids.

Zingiber Cassumunar

Roxb.

Synonym: Z. montanum (Koen.) Link ex. A. Dietr. Z. purpureum Rosc.

Family: Zingiberaceae.

Habitat: Wild throughout India, also cultivated in tropical parts of India.

English: Cassumunar Ginger.

Ayurvedic: Vanardraka, Peja, Peyu, Aardikaa, Shringaberikaa.

Folk: Adarakhi, Bana-adarakhi.

Action: Rhizome—carminative, stimulant, antispasmodic. Used for diarrhoea and colic.

The occurance of an active diastase in the rhizome has been reported.

The enzyme resembles in its activity with alpha-amylase of Aspergillus oryzae. The rhizome gave an essential oil consisting of terpinen-4-ol (about 35%); other constituents are alpha- and beta-pinene, sabinene, myrcene, terpinene, limonene, p-cymene and terpinolene. The fresh rhizomes contain phenyl butenoic dimers, along with cyclohexane derivatives. Hex- ane extract of the rhizome contained (E)-4-(3'4'-dimethoxyphenyl)-but-3- en-l-ol, known as compound D. Compound D showed antispasmodic effect on guinea-pig ileum and tracheal smooth muscle, also a dose-related uterine relaxant effect in situ in non- pregnant rats; in pregnant rats the response varied with the period of pregnancy. Aqueous extract of the rhizome showed smooth muscle relaxant and anti-inflammatory activity Antioxi- dant activity was found to be more intense than that of known curcum- inoid antioxidants. Alfalfa

Purple medick, lucerne, Californian clover, Buffalo herb. Medicago sativa L. German: Luzerne. French: Luzerne. Italian: Medica. Chinese: Mu-su. Part used: leaves.

Habitat. Native to Asia but now found in temperate zones.

Constituents. Alkaloids, isoflavones, coumarins, sterols. Contains eight essential enzymes:– amylase (digests starches), coagulase (coagulates milk), invertase (converts sugar into dextrose), emulsin (acts upon sugars), peroxidase (oxidases blood), lipase (fat-splitting enzyme), pectinase (forms a vegetable jelly from pectin), protase (digests protein). Together with its rich content of vitamins and minerals, Alfalfa offers an effective aid to cover a wide range of diverse conditions.

Vitamin content: pro-Vitamin A (B-carotene), B6, C, D, E, K, P. Yields 20,000 to 40,000 units Vitamin K to every 100 grams, and is therefore a useful preventative of high blood pressure.

Minerals. Alfalfa yields 10 times more mineral value than average grains. Roots penetrate subsoil as far as 125 feet, thus enabling it to absorb vital mineral nutrients beyond the reach of other vegetation. Calcium, Magnesium, Phosphorus, Potassium.

Action. Anti-cholesterol, anti-haemorrhagic, anti-anaemia, anti-coagulant. Traditional anti-diabetic activity (South Africa).

Uses: To promote strong bones and rebuild decayed teeth. Nutrient to increase weight and vitality. Lumbago, rheumatism, dyspepsia, back ache, fistula, chronic ulcer, infections of sinus, ear, nose and throat. Affections of respiratory tract, certain forms of insulin-dependent diabetes. Rich in chlorophyll, it stimulates growth of supportive connective tissue and is useful for collagen disease – arthritis etc. Assists granulation of tissue in healing of wounds, abscesses. Relieves constipation by gently activating peristalsis of the bowels. Frequent cups of tea have a diuretic effect relieving dropsy, kidney, bladder and prostate disorders. Fattens thin people. Builds up after surgical operation. Hyperlipidaemia. Repair of radiotherapy damage.

Preparations: Tea: 2-3 teaspoons to each cup boiling water; infuse 5-10 minutes. Drink freely.

Diuretic tea. Herbs: Alfalfa 3, Couchgrass 2, Buchu 1, Wild Carrot 1. Mix. 1-2 teaspoons to each cup boiling water; infuse 5-10 minutes. Half-1 cup thrice daily.

Liquid extract, BHP (1983), 1:1 in 25 per cent alcohol. Dose: 5-10ml thrice daily.

Green drink. Fresh Alfalfa passed through a blender; juice drunk in wineglassful doses. Alfalfa ‘sprouts’ are grown from seed in a shallow tray and contain 150 per cent more protein than wheat or corn. Daily rinsed with water, they are allowed to germinate to about half inch in height when they are chopped and used in salads. See: SPROUTS.

Any one of these preparations favourably influences nutrition in cases of anorexia nervosa, neurasthenia, insomnia, feeble appetite, and to increase the flow of milk in young nursing mothers. Taken regularly, it is believed to dispose to mental and physical well-being.

Ascites

(hydroperitoneum) n. the accumulation of fluid in the peritoneal cavity, causing abdominal swelling. Causes include infections (such as tuberculosis), heart failure, *portal hypertension, *cirrhosis, and various cancers (particularly of the ovary and liver): the presence of malignant cells in the fluid, revealed by cytological examination, is usually evidence of secondary spread. Obstruction to the drainage of lymph from the abdomen results in chylous ascites (see chyle). Pancreatic ascites, due to direct communication between the pancreatic duct and the peritoneal cavity, usually following trauma or severe pancreatitis, is diagnosed by very high amylase levels in aspirated ascitic fluid. Treatment includes diuretics and ascitic drainage (paracentesis) if there is associated respiratory distress. See also oedema.... ascites

Pancreatitis

n. inflammation of the pancreas. Acute pancreatitis is a sudden illness in which the patient experiences severe abdominal pain that radiates to the back. In severe cases, there is rapid deterioration with shock. Serum amylase levels are high. Its cause is not always discovered, but it may be associated with gallstones, alcoholism, drugs, infection, autoimmune disease, or recent interventions (such as ERCP). Complications include the formation of *pseudocysts, abscesses, necrosis (necrotizing pancreatitis), and haemorrhage (haemorrhagic pancreatitis). Treatment consists of restricting oral intake, intravenous hydration, and antibiotics if infected necrosis is present. In severe cases, *pancreatectomy may be required to remove necrosed tissue. Relapsing pancreatitis, in which the above symptoms are recurrent and less severe, may be associated with gallstones or alcoholism; prevention is by removal of gallstones and avoidance of alcohol and fat. Chronic pancreatitis may produce symptoms similar to relapsing pancreatitis or may be painless; it can lead to endocrine failure causing *malabsorption and *diabetes mellitus. The pancreas often becomes calcified, producing visible shadowing on X-rays. Autoimmune pancreatitis is a recently described condition in which an autoimmune process leads to inflammation and swelling of the pancreas. Although abdominal pain is minimal or absent, jaundice is usually present. Radiologically it is characterized by diffuse ‘sausage-shaped’ enlargement of the pancreas and narrowing of the main pancreatic duct. The presence of raised serum IgG4 is a serological marker. Treatment involves immunosuppressant agents (e.g. corticosteroids or azathioprine). Autoimmune pancreatitis is associated with other autoimmune disorders.... pancreatitis
