n. the secretion of milk by the *mammary glands of the breasts, which begins normally at the end of pregnancy or may be pathological (see galactorrhoea). A fluid called *colostrum is secreted before the milk is produced; both secretions are released in response to the sucking action of the infant on the nipple. Lactation is controlled by hormones (see prolactin; oxytocin); it stops when the baby is no longer fed at the breast.

Bromocriptine

A drug that stimulates DOPAMINE receptors in the brain. It inhibits production of the hormone PROLACTIN and is used to treat GALACTORRHOEA (excessive milk secretion) and also to suppress normal LACTATION. The drug is helpful in treating premenstrual breast engorgement and also ACROMEGALY.... bromocriptine

Menstruation

A periodic change occurring in (female) human beings and the higher apes, consisting chie?y in a ?ow of blood from the cavity of the womb (UTERUS) and associated with various slight constitutional disturbances. It begins between the ages of 12 and 15, as a rule – although its onset may be delayed until as late as 20, or it may begin as early as ten or 11. Along with its ?rst appearance, the body develops the secondary sex characteristics: for example, enlargement of the BREASTS, and characteristic hair distribution. The duration of each menstrual period varies in di?erent persons from 2– 8 days. It recurs in the great majority of cases with regularity, most commonly at intervals of 28 or 30 days, less often with intervals of 21 or 27 days, and ceasing only during pregnancy and lactation, until the age of 45 or 50 arrives, when it stops altogether – as a rule ceasing early if it has begun early, and vice versa. The ?nal stoppage is known as the MENOPAUSE or the CLIMACTERIC.

Menstruation depends upon a functioning ovary (see OVARIES) and this upon a healthy PITUITARY GLAND. The regular rhythm may depend upon a centre in the HYPOTHALAMUS, which is in close connection with the pituitary. After menstruation, the denuded uterine ENDOMETRIUM is regenerated under the in?uence of the follicular hormone, oestradiol. The epithelium of the endometrium proliferates, and about a fortnight after the beginning of menstruation great development of the endometrial glands takes place under the in?uence of progesterone, the hormone secreted by the CORPUS LUTEUM. These changes are made for the reception of the fertilised OVUM. In the absence of fertilisation the uterine endometrium breaks down in the subsequent menstrual discharge.

Disorders of menstruation In most healthy women, menstruation proceeds regularly for 30 years or more, with the exceptions connected with childbirth. In many women, however, menstruation may be absent, excessive or painful. The term amenorrhoea is applied to the condition of absent menstruation; the terms menorrhagia and metrorrhagia describe excessive menstrual loss – the former if the excess occurs at the regular periods, and the latter if it is irregular. Dysmenorrhoea is the name given to painful menstruation. AMENORRHOEA If menstruation has never occurred, the amenorrhoea is termed primary; if it ceases after having once become established it is known as secondary amenorrhoea. The only value of these terms is that some patients with either chromosomal abnormalities (see CHROMOSOMES) or malformations of the genital tract fall into the primary category. Otherwise, the age of onset of symptoms is more important.

The causes of amenorrhoea are numerous and treatment requires dealing with the primary cause. The commonest cause is pregnancy; psychological stress or eating disorders can cause amenorrhoea, as can poor nutrition or loss of weight by dieting, and any serious underlying disease such as TUBERCULOSIS or MALARIA. The excess secretion of PROLACTIN, whether this is the result of a micro-adenoma of the pituitary gland or whether it is drug induced, will cause amenorrhoea and possibly GALACTORRHOEA as well. Malfunction of the pituitary gland will result in a failure to produce the gonadotrophic hormones (see GONADOTROPHINS) with consequent amenorrhoea. Excessive production of cortisol, as in CUSHING’S SYNDROME, or of androgens (see ANDROGEN) – as in the adreno-genital syndrome or the polycystic ovary syndrome – will result in amenorrhoea. Amenorrhoea occasionally follows use of the oral contraceptive pill and may be associated with both hypothyroidism (see under THYROID GLAND, DISEASES OF) and OBESITY.

Patients should be reassured that amenorrhoea can often be successfully treated and does not necessarily affect their ability to have normal sexual relations and to conceive. When weight loss is the cause of amenorrhoea, restoration of body weight alone can result in spontaneous menstruation (see also EATING DISORDERS – Anorexia nervosa). Patients with raised concentration of serum gonadotrophin hormones have primary ovarian failure, and this is not amenable to treatment. Cyclical oestrogen/progestogen therapy will usually establish withdrawal bleeding. If the amenorrhoea is due to mild pituitary failure, menstruation may return after treatment with clomiphene, a nonsteroidal agent which competes for oestrogen receptors in the hypothalamus. The patients who are most likely to respond to clomiphene are those who have some evidence of endogenous oestrogen and gonadotrophin production. IRREGULAR MENSTRUATION This is a change from the normal monthly cycle of menstruation, the duration of bleeding or the amount of blood lost (see menorrhagia, below). Such changes may be the result of an upset in the balance of oestrogen and progesterone hormones which between them control the cycle. Cycles may be irregular after the MENARCHE and before the menopause. Unsuspected pregnancy may manifest itself as an ‘irregularity’, as can an early miscarriage (see ABORTION). Disorders of the uterus, ovaries or organs in the pelvic cavity can also cause irregular menstruation. Women with the condition should seek medical advice. MENORRHAGIA Abnormal bleeding from the uterus during menstruation. A woman loses on average about 60 ml of blood during her period; in menorrhagia this can rise to 100 ml. Some women have this problem occasionally, some quite frequently and others never. One cause is an imbalance of progesterone and oestrogen hormones which between them control menstruation: the result is an abnormal increase in the lining (endometrium) of the uterus, which increases the amount of ‘bleeding’ tissue. Other causes include ?broids, polyps, pelvic infection or an intrauterine contraceptive device (IUD – see under CONTRACEPTION). Sometimes no physical reason for menorrhagia can be identi?ed.

Treatment of the disorder will depend on how severe the loss of blood is (some women will become anaemic – see ANAEMIA – and require iron-replacement therapy); the woman’s age; the cause of heavy bleeding; and whether or not she wants children. An increase in menstrual bleeding may occur in the months before the menopause, in which case time may produce a cure. Medical or surgical treatments are available. Non-steroidal anti-in?ammatory drugs may help, as may tranexamic acid, which prevents the breakdown of blood clots in the circulation (FIBRINOLYSIS): this drug can be helpful if an IUD is causing bleeding. Hormones such as dydrogesterone (by mouth) may cure the condition, as may an IUD that releases small quantities of a PROGESTOGEN into the lining of the womb.

Traditionally, surgical intervention was either dilatation and curettage of the womb lining (D & C) or removal of the whole uterus (HYSTERECTOMY). Most surgery is now done using minimally invasive techniques. These do not require the abdomen to be cut open, as an ENDOSCOPE is passed via the vagina into the uterus. Using DIATHERMY or a laser, the surgeon then removes the whole lining of the womb. DYSMENORRHOEA This varies from discomfort to serious pain, and sometimes includes vomiting and general malaise. Anaemia is sometimes a cause of painful menstruation as well as of stoppage of this function.

In?ammation of the uterus, ovaries or FALLOPIAN TUBES is a common cause of dysmenorrhoea which comes on for the ?rst time late in life, especially when the trouble follows the birth of a child. In this case the pain exists more or less at all times, but is aggravated at the periods. Treatment with analgesics and remedying the underlying cause is called for.

Many cases of dysmenorrhoea appear with the beginning of menstrual life, and accompany every period. It has been estimated that 5–10 per cent of girls in their late teens or early 20s are severely incapacitated by dysmenorrhoea for several hours each month. Various causes have been suggested for the pain, one being an excessive production of PROSTAGLANDINS. There may be a psychological factor in some sufferers and, whether this is the result of inadequate sex instruction, fear, family, school or work problems, it is important to o?er advice and support, which in itself may resolve the dysmenorrhoea. Symptomatic relief is of value.... menstruation

Breast

Either 1 of the 2 mammary glands, which, in women, provide milk to nourish a baby and are secondary sexual characteristics. In males, the breast is an immature version of the female breast. At puberty, a girl’s breasts begin to develop: the areola (the circular area of pigmented skin around the nipple) swells and the nipple enlarges. This is followed by an increase in glandular tissue and fat. The adult female breast consists of 15–20 lobes of milk-secreting glands embedded in fatty tissue. The ducts of these glands have their outlet in the nipple. Bands of fine ligaments determine the breast’s height and shape. The areolar skin contains sweat glands, sebaceous glands, and hair follicles.

The size and shape and general appearance of the breasts may vary during the menstrual cycle, during pregnancy and lactation, and after the menopause.

During pregnancy, oestrogen and progesterone, secreted by the ovary and placenta, cause the milkproducing glands to develop and become active and the nipple to become larger.

Just before and after

childbirth, the glands in the breast produce a watery fluid known as colostrum.

This fluid is replaced by milk a few days later.

Aguacate

Avocado (Persea americana).

Plant Part Used: Leaves, seed, fruit.

Dominican Medicinal Uses: The leaves are traditionally prepared as an infusion and taken orally for diabetes, diarrhea, inducing abortion, intestinal worms, menstrual cramps, parasites and vaginal infections, and the seed decoction is taken for contraception. The fruit is typically used for nutritional and culinary purposes.

Safety: No data on the safety of the leaf or the seed in humans has been identified in the available literature; animal toxicity studies have shown equivocal results. The fruit is commonly consumed as food and generally regarded as safe.

Contraindications: Oral use of the leaves is contraindicated during pregnancy (due to emmenagogue and uterine muscle stimulating effects) and lactation (due to potential for harmful effects based on case reports in goats). No information on the safety of the leaves in children has been identified in the available literature.

Drug Interactions: Warfarin: fruit may inhibit anticoagulant effect. Monoamine-oxidase inhibitors (MAOI): one case of hypertension crisis has been reported due to concomitant ingestion of the fruit and MAOI.

Clinical Data: The following effects of this plant have been investigated in human clinical trials: fruit: cholesterol and lipid-lowering, treatment of non-insulin dependent diabetes mellitus and triglyceride-lowering; avocado/soybean unsaponifiables: treatment of osteoarthritis; and oil: treatment of plaque psoriasis.

Laboratory & Preclinical Data: The following biological activities of this plant have been investigated in laboratory and preclinical studies (in vitro or animal models): analgesic, anti-inflammatory, antihemorrhage, hepatoprotective, immuno-modulating, uterine muscle stimulant, trypanocidal, uterine stimulant and vasorelaxant.

Ají

Pepper, bell pepper, chili pepper, cayenne (Capsicum annuum, C. frutescens & C. chinense).

Plant Part Used: Leaf, fruit.

Dominican Medicinal Uses: The leaf is traditionally prepared as a warm poultice and applied topically for skin abscesses, boils or infections, or prepared as a tea and taken orally for menstrual cramps and related disorders. The fruit is typically used for culinary and nutritional purposes and is said to increase heat in the body.

Safety: No data on the safety of the leaf in humans (for internal or external use) has been identified in the available literature; however, in animal studies, topical application of the leaf did not show signs of toxicity or adverse effects. The fruit is widely consumed and considered safe in moderate amounts. Prolonged or excessive use may cause irritation of the mucosa or other adverse effects.

Contraindications: No data on the safety of this plant during pregnancy, lactation or in children has been identified in the available literature. The fruit should not be taken by patients with inflammatory gastro-intestinal or renal disorders. Avoid contact with the eyes or open wounds due to potential irritation of the mucosa.

Drug Interactions: Consumption of the fruit may inhibit liver microsomal enzymes and potentiate drugs metabolized by these enzymes. Aspirin and salicylic acid compounds: bioavailability may be reduced by concurrent use of peppers. Barbiturates: concomitant use of the dried fruit has been shown to potentiate the effects of hexobarbital. Anticoagulants, antiplatelet agents, thrombolytic agents: concomitant use of the fruit may increase the risk of bleeding.

Clinical Data: No human clinical trials of the leaf have been identified in the available literature. The fruit has been investigated in clinical trials for the following effects: analgesic, carotenoid bioavailability enhancement, gastroprotective, swallowing dysfunction treatment and urinary incontinence treatment.

Ajo

Garlic (Allium sativum).

Plant Part Used: Bulb.

Dominican Medicinal Uses: The bulb is traditionally ingested raw for high blood pressure, upper-respiratory infection, common cold, flu-like symptoms and cough, and the alcohol extract is taken internally for sinusitis. The bulb skins are traditionally prepared as a tea and taken internally for indigestion and gastro-intestinal complaints.

Safety: The bulb is generally regarded as safe for human consumption. Reported adverse effects include skin burns due to topical application (especially in children with prolonged exposure). Adverse effects associated with internal use include halitosis, body odor, gastrointestinal irritation, constipation, headache, nausea, fatigue and vertigo.

Contraindications: Not to be taken at therapeutic doses for 10 days prior to surgery due to antiplatelet activity and risk of excessive bleeding. The bulb is contraindicated during lactation.

Drug Interactions: Chlorzoxazone: garlic may reduce drug metabolism. Indomethacin and NSAIDs: risk of excessive bleeding. Protease inhibitors: reduced blood levels. Drugs metabolized by cytochrome P450 2E1: garlic may inhibit efficacy. Forskilin: garlic may potentiate antiplatelet activity.

Clinical Data: The following effects have been investigated in human clinical trials: treatment of atherosclerosis, common cold, coronary artery disease, hyperlipidemia, hypertension and unstable angina pectoris.

Cod-liver Oil

Cod-liver oil is derived from the fresh liver of the cod (Gadus callarius). It is a rich source of vitamin D, used in the prevention and treatment of RICKETS, and of vitamin A. Human milk contains more than enough vitamin D for the breast-fed baby, provided the mother has a balanced diet with adequate exposure to sunlight, or is taking vitamin supplements during pregnancy and lactation if considered necessary. All baby foods in the UK contain added vitamins, and therefore supplementation is unnecessary until weaning begins, and the baby starts taking cow’s milk, which contains less vitamin D than human milk. Albahaca

Basil (Ocimum basilicum).

Plant Part Used: Aerial parts: leaf, stem, flower.

Dominican Medicinal Uses: The aerial parts or leaves are traditionally prepared as a tea and taken orally for stomach ache, indigestion, gastro-intestinal pain, internal cleansing and women’s health conditions.

Safety: This herb is generally regarded as safe for human consumption in moderate amounts and widely used as a culinary seasoning.

Contraindications: The essential oil should not be used during pregnancy, lactation or in small children.

Drug Interactions: Synergistic effects may occur with drugs that share similar pharmacological activities as those described for this plant in the “Laboratory and Preclinical Data” section; metabolism of one of basil’s active constituents, estragole, may be hindered by concomitant use of medications metabolized by UGT2B7 or UGT1A9 phase II enzymes.

Clinical Data: No human clinical trials of this plant have been identified in the available literature.

Alcanfor

Camphor (Cinnamomum camphora).

Plant Part Used: Essential oil.

Dominican Medicinal Uses: The crystallized essential oil is traditionally prepared as an ointment and applied topically for treating sinusitis, headache, upper-respiratory tract infections, muscle pain, joint pain, asthma, bronchitis, difficulty breathing and phlegm in the lungs. For internal use, a small amount of the essential oil is dissolved in water and taken orally for gas, indigestion and stomach ache.

Safety: Internal use of the essential oil can be highly toxic (adult lethal dose = 20 g; toxic at 2 g; child lethal dose < 1 g). External use may cause skin irritation. Overdose symptoms include: delirium, spasms, intoxicated states and irregular breathing.

Contraindications: Caution advised when administered topically to children, and external use is contraindicated in cases of broken skin. In infants and small children (< 2 years), the oil should not be administered near the nose or via inhalation due to potential nervous system overstimulation or possibility of seizures. Avoid internal use during pregnancy (due to emmenagogue and uterine stimulant effects) and lactation (due to potential toxicity).

Clinical Data: The following effects of the essential oil have been investigated in human clinical trials: nasal sensation of cold, central nervous system stimulant, antiplatelet, Demodex rosacea treatment and ophthalmic disorder treatment.

Laboratory & Preclinical Data: The following biological activities of the essential oil or its constituents have been investigated using in vitro or animal models: anti-inflammatory, antioxidant, biosurfactant, cytotoxic, positively inotropic, ribosome inactivation, smooth muscle stimulant and superoxide dismutase.

Altamisa

Ragweed (Ambrosia artemisiifolia and A. peruviana).

Plant Part Used: Leaf, aerial parts.

Dominican Medicinal Uses: The leaf is traditionally prepared as a tea and taken orally for arthritis, delayed menses, diarrhea (in children and adults), infections, kidney ailments, menstrual pain, postpartum cleansing and stomach ache. It is also used externally as a poultice for menstrual pain and as a bath for energetic cleansing, good luck and spiritual protection.

Safety: The pollen of Ambrosia species is a common allergen and may cause symptoms of hayfever in hypersensitive individuals. Plant material should be washed thoroughly before use to remove pollen. Cases of contact dermatitis, eczema, allergic conjunctivitis and other adverse effects have been associated with this plant.

Contraindications: Due to lack of available safety information, avoid use during pregnancy or lactation and in children under 5 years of age.

Drug Interactions: Synergistic interactions may occur with medications that share similar biological activities to those demonstrated by this herb (see “Clinical Data” and “Laboratory & Preclinical Data” below).

Gymnema Hirsutum

Wight & Arn.

Family: Asclepiadaceae.

Habitat: Southern parts of Uttar Pradesh adjoining Madhya Pradesh, also in Bihar and Western Ghats.

Folk: Gurmaar (related species).

Action: Leaf—when chewed, temporarily paralyses the sense of taste for sweet and bitter substances. Plant—used as stomachic, bechic, expectorant, and in male impotency, poor lactation.

The leaves contain gymnemic acid.... gymnema hirsutum

Medicinal Yams

Dioscorea spp.

Dioscoreaceae

The growing need for steroidal drugs and the high cost of obtaining them from animal sources led to a widespread search for plant sources of steroidal sapogenins, which ultimately led to the most promising one. It is the largest genus of the family constituted by 600 species of predominantly twining herbs. Among the twining species, some species twine clockwise while others anti-clockwise (Miege, 1958). All the species are dioceous and rhizomatous. According to Coursey (1967), this genus is named in honour of the Greek physician Pedenios Dioscorides, the author of the classical Materia Medica Libri Quinque. Some of the species like D. alata and D. esculenta have been under cultivation for a long time for their edible tubers. There are about 15 species of this genus containing diosgenin. Some of them are the following (Chopra et al, 1980).

Among the above said species, D. floribunda, D. composita and D. deltoidea are widely grown for diosgenin production.

1. D. floribunda Mart. & Gal D. floribunda Mart. & Gal. is an introduction from central America and had wide adaptation as it is successfully grown in Karnataka, Assam, Meghalaya, Andaman and Goa. The vines are glabrous and left twining. The alternate leaves are borne on slender stems and have broadly ovate or triangular ovate, shallowly cordate, coriaceous lamina with 9 nerves. The petioles are 5-7cm long, thick and firm. Variegation in leaves occurs in varying degrees. The male flowers are solitary and rarely in pairs. Female flowers have divericate stigma which is bifid at apex. The capsule is obovate and seed is winged all round. The tubers are thick with yellow coloured flesh, branched and growing upto a depth of 30cm (Chadha et al, 1995).

2. D. composita Hemsl.

D. composita Hemsl. according to Knuth (1965) has the valid botanical name as D. macrostachya Benth. However, D. composita is widely used in published literature. It is a Central American introduction into Goa, Jammu, Bangalore, Anaimalai Hills of Tami Nadu and Darjeeling in W. Bengal. The vines are right twinning and nearly glabrous. The alternate leaves have long petioles, membraneous or coriaceous lamina measuring upto 20x18cm, abruptly acute or cuspidate-acuminate, shallowly or deeply cordate, 7-9 nerved. The fasciculate-glomerate inflorescence is single or branched with 2 or 3 sessile male flowers having fertile stamens. Male fascicle is 15-30cm long. The female flowers have bifid stigma. Tubers are large, white and deep-rooted (upto 45cm) (Chadha et al, 1995).

3. D. deltoidea Wall. ex. Griseb.

D. deltoidea Wall. ex. Griseb. is distributed throughout the Himalayas at altitudes of 1000-3000m extending over the states of Jammu-Kashmir, H. P, U. P, Sikkim and further into parts of W. Bengal. The glabrous and left twining stem bears alternate petiolate leaves. The petioles are 5-12 cm long. The lamina is 5-15cm long and 4-12cm wide widely cordate. The flowers are borne on axillary spikes, male spikes 8-40cm long and stamens 6. Female spikes are 15cm long, 3. 5cm broad and 4-6 seeded. Seeds are winged all round. Rhizomes are lodged in soil, superficial, horizontal, tuberous, digitate and chestnut brown in colour (Chadha et al, 1995). D. deltoidea tuber grows parallel to ground covered by small scale leaves and is described as rhizome. The tubers are morphologically cauline in structure with a ring of vascular bundles in young tubers which appear scattered in mature tubers (Purnima and Srivastava, 1988). Visible buds are present unlike in D. floribunda and D. composita where the buds are confined to the crown position (Selvaraj et al, 1972).

Importance of Diosgenin: Diosgenin is the most important sapogenin used as a starting material for synthesis of a number of steroidal drugs. For commercial purposes, its -isomer, yamogenin is also taken as diosgenin while analysing the sample for processing. Various steroidal drugs derived from diosgenin by artificial synthesis include corticosteroids, sex hormones, anabolic steroids and oral contraceptives. Corticosteroids are the most important group of steroidal drugs synthesized from diosgenin. First group of corticosteroids regulates carbohydrate and protein metabolism. The second group consists of aldosterone, which controls balance of potassium, sodium and water in the human body. The glucocorticoids in the form of cortisone and hydrocortisone are used orally, intramuscularly or topically for treatment of rheumatoid arthritis, rheumatic fever, other collegen diseases, ulcerative colitis, certain cases of asthma and a number of allergic diseases affecting skin, eye and the ear. These are also used for treatment of gout and a variety of inflammations of skin, eye and ear and as replacement therapy in Addison’s diseases. The minerato corticoides, desoxycorticosterone or desoxycortone are used in restoring kidney functions in cases of cortical deficiency and Addison’s disease.

Both male and female sex hormones are also synthesized from disosgenin. The main male sex hormone (androgen) which is produced from disogenin is testosterone. The main female sex hormones produced are oestrogen and progesterone. Recently oestrogen has also been used in cosmetic lotions and creams to improve the tone and colour of skin. One of the main uses of progesterone during recent years has been as antifertility agent for oral contraceptives. These artificial steroids have increased oral activity and fewer side effects, as they can be used in reduced doses. Oral contraceptives are also used for animals like pigs, cows and sheep to control fertility and to give birth at a prescribed period in a group of animals at the same time. These compounds are also used to reduce the interval between the lactation periods to have more milk and meat production. Anti-fertility compounds are also used as a pest-control measure for decreasing the multiplication of pests like rodents, pigeons and sea gulls (Husain et al, 1979).

Although yam tubers contain a variety of chemical substances including carbohydrates, proteins, alkaloids and tannins, the most important constituents of these yams are a group of saponins which yield sapogenins on hydrolysis. The most imp ortant sapogenin found in Dioscorea are diosgenin, yamogenin and pannogenin. Diosgenin is a steroid drug precursor. The diogenin content varies from 2-7% depending on the age of the tubers. Saponins including 5 spirastanol glucoside and 2 furostanol glucoside, 4 new steroid saponins, floribunda saponins C, D, E and F. Strain of A and B are obtained from D. floribunda (Husain et al, 1979). Rhizomes of D. deltoidea are a rich source of diosgenin and its glycoside. Epismilagenin and smilagenone have been isolated from D. deltoidea and D. prazeri (Chakravarti et al, 1960; 1962). An alkaloid dioscorine has been known to occur in D. hispida (Bhide et al,1978). Saponin of D. prazeri produced a fall of blood pressure when given intravenously and saponin of D. deltoidea has no effect on blood pressure (Chakravarti et al,1963). Deltonin, a steroidal glycoside, isolated from rhizomes of D. deltoidea showed contraceptive activity (Biokova et al, 1990).

Agrotechnology: Dioscorea species prefer a tropical climate without extremity in temperature. It is adapted to moderate to heavy rainfall area. Dioscorea plants can be grown in a variety of soils, but light soil is good, as harvesting of tubers is easier in such soils. The ideal soil pH is 5.5-6.5 but tolerates fairly wide variation in soil pH. Dioscorea can be propagated by tuber pieces, single node stem cuttings or seed. Commercial planting is normally established by tuber pieces only. Propagation through seed progeny is variable and it may take longer time to obtain tuber yields. IIHR, Bangalore has released two improved varieties, FB(c) -1, a vigorously growing strain relatively free from diseases and Arka Upkar, a high yielding clone. Three types of tuber pieces can be distinguished for propagation purpose, viz. (1) crown (2) median and (3) tip, of which crowns produce new shoots within 30 days and are therefore preferred. Dipping of tuber pieces for 5 minutes in 0.3% solution of Benlate followed by dusting the cut ends with 0.3% Benlate in talcum powder in mo ist sand beds effectively checks the tuber rot. The treatment is very essential for obtaining uniform stand of the crop. The best time of planting is the end of April so that new sprouts will grow vigorously during the rainy season commencing in June in India. Land is to be prepared thoroughly until a fine tilth is obtained. Deep furrows are made at 60cm distance with the help of a plough. The stored tuber pieces which are ready for planting is to be planted in furrows with 30cm between the plants for one year crop and 45cm between the plants for 2 year crop at about 0.5 cm below soil level. The new sprouts are to be staked immediately. After sprouting is complete, the plants are to be earthed up. Soil from the ridges may be used for earthing up so that the original furrows will become ridges and vice versa. Dioscorea requires high organic matter for good tuber formation. Besides a basal doze of 18-20t of FYM/ha, a complete fertilizer dose of 300kg N, 150kg P2O5 and K2O each are to be applied per hectare. P and K are to be applied in two equal doses one after the establishment of the crop during May-June and the other during vigorous growth period of the crop (August- September). Irrigation may be given at weekly intervals in the initial stage and afterwards at about 10 days interval. Dioscorea vines need support for their optimum growth and hence the vines are to be trailed over pandal system or trellis. Periodic hand weeding is essential for the first few months. Intercropping with legumes has been found to smother weeds and provide extra income. The major pests of Dioscorea are the aphids and red spider mites. Aphids occur more commonly on young seedlings and vines. Young leaves and vine tips eventually die if aphids are not controlled. Red spider mites attack the underside of the leaves at the base near the petiole. Severe infestations result in necrotic areas, which are often attacked by fungi. Both aphids and spider mites can be very easily controlled by Kelthane. No serious disease is reported to infect this crop. The tubers grow to about 25-30 cm depth and hence harvesting is to be done by manual labour. The best season for harvesting is Feb-March, coinciding with the dry period. On an average 50-60t/ha of fresh tubers can be obtained in 2 years duration. Diosgenin content tends to increase with age, 2.5% in first year and 3-3.5% in the second year. Riboflavin

The British Pharmacopoeia name for what used to be known as vitamin B2. The minimal daily requirement for an adult is 1·5–3 mg, but is greater during pregnancy and lactation. Tobacco (Nicotiana tabacum).

Alucema

Lavender (Lavandula angustifolia).

Plant Part Used: Dried flower buds.

Dominican Medicinal Uses: The dried flower buds are traditionally prepared as a tea and taken orally for anxiety/nervousness, stomach ache, indigestion, gas, menopausal hot flashes, common cold and flu.

Safety: Lavender is generally regarded as safe when used in moderation. Potential adverse effects include drowsiness, gastrointestinal upset and skin irritation.

Contraindications: Excessive internal use of this herb is contraindicated during early pregnancy due to its emmenagogue effect demonstrated in laboratory studies. Due to lack of sufficient data on safety, avoid use during lactation and in small children.

Drug Interactions: Concomitant use of this herb with sedative or tranquilizing drugs, such as pentobarbital, may potentiate their effects based on evidence from animal studies. Additional herb-drug interactions may occur in medications with effects similar to those demonstrated by this plant clinical and preclinical studies (see below).

Clinical Data: The following effects of this plant have been investigated in human clinical trials: antianxiety, antidepressant, anti-stress, anxiolytic, dysmenorrhea treatment, hypnotic, insomnia treatment, retrospective pain perception and sedative.

Anamú

Guinea-hen-weed (Petiveria alliacea).

Plant Part Used: Leaf, root, stem.

Dominican Medicinal Uses: The root is traditionally prepared as a tincture in alcohol and taken orally for arthritis, joint and muscle pain. The leaf and/or root is typically prepared as an infusion and taken orally for nausea and stomach ailments, women’s health conditions (dysmenorrhea, menorrhagia, menopausal symptoms, ovarian cysts, labor pains, postpartum recovery, uterine fibroids) and to cleanse the blood. The leaf is also prepared as a poultice and applied topically for skin infections.

Safety: No data on the safety of this plant in humans has been identified in the available literature. Animal studies of the leaf have shown relatively low toxicity, and TRAMIL has approved this herb for particular traditional uses.

Contraindications: Avoid use during pregnancy, lactation and in children under 12 years of age.

Drug Interactions: Concomitant use of this herb with insulin and hypoglycemic drugs may potentiate their effects.

Clinical Data: No human clinical trials have been identified in the available literature.

Anís Chiquito

Anise, anise burnet-saxifrage (Pimpinella anisum).

Plant Part Used: Fruit (seed).

Dominican Medicinal Uses: The seeds are traditionally prepared as a decoction and taken orally for colic (in children and adults), common cold, empacho, flatulence, flu, gastrointestinal disorders, headache, indigestion, nervous tension, pasmo and stress.

Safety: The seeds are generally regarded as safe for human consumption in moderation and widely used as a culinary spice. Caution is advised if this herbal remedy is combined with anís de estrella due to potential contamination with a toxic look-alike (see entry for “Anís de estrella”).

Contraindications: Studies show conflicting recommendations regarding safety of internal use during pregnancy and lactation. Use of this herb in combination with anís de estrella is contraindicated in children (due to potential for contamination with the toxic look-alike Illicium anisatum (see “Anís de estrella”); however, anís chiquito is considered safe for children when used appropriately.

Drug interactions: Anticoagulants, NSAIDS, antiplatelet drugs, warfarin: Avoid use of anís chiquito if taking any of these medications due to potential risk of excessive bleeding as a result of interaction with coumarin derivatives.

Clinical Data: No clinical trials of the oral use of this herb have been identified in the available literature. One open clinical trial has evaluated the pediculicidal effects of anise oil in combination with other ingredients.

Cancer - Breast

Commonest form of cancer in women. Overall mortality remains about 50 per cent at five years. Appears to run in families. Strikes hard unmarried women. Married women who have no children. Those who do not nurse their babies, or who are infertile and have no child before thirty. Eight out of ten chest lumps are benign.

Symptoms. A small lump comes to light while washing, a discharge from the nipple, change in nipple size and colour, irregular contour of the breast surface. Though tissue change is likely to be a cyst, speedy diagnosis and treatment are necessary. Some hospital physicians and surgeons are known to view favourably supportive herbal aids, and do not always think in terms of radical mastectomy. Dr Finlay Ellingwood, Chicago physician (1916) cured a case by injection of one dram Echinacea root extract twice a week into the surrounding tissues.

The condition is believed to be due to a number of causes including suppression of ovulation and oestrogen secretion in pregnant and lactating women. A high fat diet is suspected of interference with the production of oestrogen. Some women are constitutionally disposed to the condition which may be triggered by trauma or emotional shock. Increase in incidence in older women has been linked with excessive sugar consumption. “Consumption overwhelms the pancreas which has to ‘push it out’ to all parts of the body (when broken down by the digestive process) whether they need it or not. The vital organs are rationed according to their requirements of nutrients from the diet. What is left over has to ‘go into store elsewhere’. And the breast is forced to take its share and store it. If it gets too much, for too long, it may rebel!” (Stephen Seely, Department of Bacteriology and Virology, Manchester)

“Women who nurse their babies less than one month are at an increased risk for breast cancer. The longer a woman breast-feeds – no matter what her age – the more the risk decreases. (Marion Tompson, co-founder, The La Leche League, in the American Journal of Epidemiology)

Lactation reduces the risk of pre-menopausal breast cancer. (Newcomb P.A. et al New England Journal of Medicine, 330 1994)

There is currently no treatment to cure metastatic breast cancer. In spite of chemotherapy, surgery and radiotherapy survival rate has not diminished. Herbs not only have a palliative effect but, through their action on hormone function offer a positive contribution towards overcoming the condition. Their activity has been widely recorded in medical literature. Unlike cytotoxic drugs, few have been known to cause alopecia, nausea, vomiting or inflammation of the stomach.

Treatment by a general medical practitioner or oncologist.

Special investigations. Low radiation X-ray mammography to confirm diagnosis. Test for detection of oestrogen receptor protein.

Treatment. Surgery may be necessary. Some patients may opt out from strong personal conviction, choosing a rigid self-disciplined approach – the Gentle Way. Every effort is made to build up the body’s natural defences (immune system).

An older generation of herbalists believed tissue change could follow a bruise on the breast, which should not be neglected but immediately painted with Tincture Arnica or Tincture Bellis perennis.

Vincristine, an alkaloid from Vinca rosea (Catharanthus roseus) is used by the medical profession as an anti-neoplastic and anti-mitotic agent to inhibit cell division.

Of possible therapeutic value. Blue Flag root, Burdock root, Chaparral, Clivers, Comfrey root, Echinacea, Figwort, Gotu Kola, Marshmallow root, Mistletoe, Myrrh, Prickly Ash bark, Red Clover, Thuja, Wild Violet, Yellow Dock.

Tea. Equal parts: Red Clover, Clivers, Gotu Kola, Wild Violet. 1 heaped teaspoon to each cup boiling water; infuse 5-15 minutes. 3 or more cups daily.

Decoctions. Echinacea, Blue Flag root, Queen’s Delight, Yellow Dock.

Tablets/capsules. Blue Flag root, Echinacea, Poke root, Mistletoe.

Formula. Echinacea 2; Gotu Kola 1; Poke root 1; Mistletoe 1; Vinca rosea 1. Mix. Dose: Powders: 500mg (two 00 capsules or one-third teaspoon). Liquid extracts: 1 teaspoon. Tinctures: 2 teaspoons. Thrice daily and at bedtime. According to progress of the disease, increase dosage as tolerated.

Maria Treben’s tea. Parts: Marigold (3), Yarrow 1; Nettles 1. Mix. 2 teaspoons to each cup boiling water. 1 cup as many times daily as tolerated.

William Boericke, M.D. recommends Houseleek. E.H. Ruddock M.D. favours Figwort.

Topical. Treatments believed to be of therapeutic value or for use as a soothing application.

(1) Cold poultice: Comfrey root.

(2) Poultice of fresh Marshmallow root pulped in juicer.

(3) Injection of Extract Greater Celandine (Chelidonium), locally, gained a reputation in the Eclectic school.

(4) The action of Blood root (Sanguinaria) is well known as a paint or injection.

(5) Ragwort poultice: 2oz Ragwort boiled in half a pint potato water for 15 minutes. See: POULTICE.

(6) Popular Russian traditional remedy: Badiaga (Spongilla fluviatilis), fresh water sponge gathered in the autumn; dried plant rubbed to a powder. Poultice.

(7) Maria Treben’s Poultice: Carefully washed fresh Plantain leaves, pulped, and applied direct to the lesion.

(8) If lymph glands are affected, apply Plantain poultice to glands.

(9) Dr Brandini’s treatment. Dr Brandini, Florence, used 4 grains Citric Acid (prepared from lemons) in 1oz (30ml) water for ulcerated cancer of the breast considered incurable. “The woman’s torments were so distressing that neither she nor other patients could get any rest. Applying lint soaked in the solution, relief was instantaneous. Repeated, it was successful.”

(10) Circuta leaves. Simmered till soft and mixed with Slippery Elm bark powder as a poultice morning and night.

(11) Decoction. Simmer gently Yellow Dock roots, fine cut or powdered, 1oz to 1 pint, 20 minutes. Saturate lint or suitable material and apply.

(12) Yellow Dock ointment. Half ounce Lobelia seed, half ounce Yellow Dock root powder. Baste into an ointment base. See: OINTMENT BASE.

(13) Infusion, for use as a wash. Equal parts: Horsetail, Red Clover, Raspberry leaves. 1oz to 1 pint boiling water infuse 15 minutes.

(14) Dr Christopher’s Ointment. Half an ounce White Oak, half an ounce Garden Sage, half an ounce Tormentil, half an ounce Horsetail, half an ounce Lemon Balm. Method: Boil gently half an hour in quart water, strain. Reduce to half a pint by simmering. Add half a pound honey. Bring to boil. Skim off scum. Allow cool. Apply: twice daily on sores.

(15) Dr Finlay Ellingwood. Poke root juice. “Fresh juice from the stems, leaves and roots applied directly to diseased tissue. Exercises a selective action; induces liquefaction and promotes removal, sometimes healing the open wound and encouraging scar formation. Masses of such tissue have been known to be destroyed in a few weeks with only a scar, with no other application but the fresh juice. Produces pain at first, but is otherwise harmless.”

(17) Dust affected parts with Comfrey powder. Mucilage from Comfrey powder or crushed root with the aid of a little milk. See: COMFREY.

(18) Dr Samuel Thomson’s Cancer Plaster. “Take heads of Red Clover and fill a kettle. Boil in water for one hour. Remove and fill kettle with fresh flower heads. Boil as before in the same liquor. Strain and press heads to express all the liquor. Simmer over a low fire till of the consistency of tar. It must not burn. Spread over a piece of suitable material.”

(20) Chinese Herbalism. Take 1-2 Liang pulverised liao-ko-wang (Wickstroemia indica), mix with cold boiled water or rice wine for local compress. Also good for mastitis.

(21) Italian women once used an old traditional remedy – Fenugreek tea.

(22) A clinical trial of Vitamin D provided encouraging results. Patients with locally advanced breast cancer were given a highly active Vitamin D analogue cream to rub on their tumours. “It was effective in one third of the tumours,” said Professor Charles Coombes, clinical oncologist, Charing Cross Hospital, London.

Diet. “A diet rich in cereal products (high in dietary fibre) and green leafy vegetables (antioxidants) would appear to offer women some protection against breast cancer due to the relation between fibre and oestrogen metabolism. Meat-free diet. In a study of 75 adolescent girls, vegetarians were found to have higher levels of a hormone that women suffering from breast cancer often lack. (Cancer Research) Supplements. Daily. Chromium. Selenium (600mcg). Zinc chelate (100mg morning and evening). Beta carotene. “Low levels of Selenium and Vitamins A and E are shown in breast cancer cases.” (British Journal of Cancer 49: 321-324, 1984).

Vitamins A and D inhibit virus penetration in healthy cell walls. Multivitamin combinations should not include Vitamin B12, production of which in the body is much increased in cancerous conditions. Vitamins B-complex and C especially required.

Note: A link between sugar consumption and breast cancer has been reported by some authorities who suggest that countries at the top of the mortality table are the highest also in sugar consumption; the operative factor believed to be insulin.

Screening. Breast screening should be annual from the age of forty.

General. Mothers are encouraged to breast-feed children for the protection it offers against mammary malignancy. (Am.J. Obstet. Gyn. 15/9/1984. 150.)

n. a *trace element that is a cofactor of many enzymes: it is important in the immune response and has structural properties in some proteins. Avena

Plant Part Used: Seeds (oat grain), fruiting tops.

Dominican Medicinal Uses: Oats are traditionally boiled in water to make oatmeal or an oatmeal-like beverage and taken orally for high cholesterol, to stimulate lactation, for nutrition and strength and to relieve menopausal hot flashes.

Safety: Oats are commonly consumed and generally regarded as safe. They have shown low potential for allergic reaction in gluten-sensitive individuals.

Contraindications: In patients with celiac disease, oats may cause gastrointestinal irritation, but they have been shown to be well-tolerated in recent clinical studies.

Drug Interactions: Lovastatin and statin drugs (impaired absorption of HMG-CoA reductase inhibitors).

Clinical Data: The following effects of oats or oat extracts have been investigated in human clinical trials: anti-diabetic, cholesterol-lowering, hypoglycemic, hypocholesterolemic, smoking cessation (grain extract or oat bran); antihyperlipidemic, antihypertensive, reduced heart disease risk, stimulation of bile acid secretion and synthesis, tolerance in celiac patients (whole-grain and oat bran); anti-skin irritant, burn wound-healing, itch reduction (topical oil-based preparation).

Barley Tea May Fight Cancer

Barley tea is widely consumed due to its medicinal properties. It fights effectively against several types of cancer, due to its high content of antioxidants. Barley Tea description Barley is a self-pollinating annual plant, member of the grass family. It grows to a height of 1 to 4 feet, being able to withstand various growing conditions. It is found in grasslands, woodlands, disturbed habitats, roadsides and orchards. The grass of barley is acknowledged to be a source of fiber, vitamins, minerals, and amino acids and it also has a high content of antioxidants. In traditional Chinese medicine, Barley grass has been prescribed to fight diseases of the spleen or poor digestion. It has also been effectively used to treat depression or emotional imbalance. Breasts

Breasts, or mammary glands, occur only in mammals and provide milk for feeding the young. These paired organs are usually fully developed only in adult females, but are present in rudimentary form in juveniles and males. In women, the two breasts over-lie the second to sixth ribs on the front of the chest. On the surface of each breast is a central pink disc called the areola, which surrounds the nipple. Inside, the breast consists of fat, supporting tissue and glandular tissue, which is the part that produces milk following childbirth. Each breast consists of 12–20 compartments arranged radially around the nipple: each compartment opens on to the tip of the nipple via its own duct through which the milk ?ows. The breast enlargement that occurs in pregnancy is due to development of the glandular part in preparation for lactation. In women beyond childbearing age, the glandular part of the breasts reduces (called involution) and the breasts become less ?rm and contain relatively more fat.

Bruja

Life plant (Kalanchoe pinnata).

Note: this name can also refer to: Mala madre. Distinguishing feature: bruja leaves are shorter than those of mala madre.

Plant Part Used: Leaf.

Dominican Medicinal Uses: The leaves are traditionally heated until wilted and squeezed to extract the juice from inside the leaf which is applied topically for earache. The bruised, fresh leaves are also applied topically for headache, and the fresh leaves or leaf juice are taken orally for stomach ache and ulcers.

Safety: In a clinical case report, the leaf extract (30 g fresh leaves per day taken orally for 14 days) did not show any signs of toxicity or adverse effects in one adult female patient. The leaf orally administered to mice for 30 days did not show signs of toxicity to the liver, heart or kidney.

Contraindications: No information has been identified in the available literature on the safety of this plant in children or during pregnancy or lactation.

Clinical Data: In one clinical case report the leaf extract was investigated for its potential in treating leishmaniasis.

Laboratory & Preclinical Data: In animal studies the leaf extract has shown antitumor effects and the leaf juice has shown hepatoprotective activity. In vitro, the leaf extract or constituents have demonstrated antitumor and uterine stimulant effects.

Café

Coffee (Coffea arabica).

Plant Part Used: Seed, leaf.

Dominican Medicinal Uses: The roasted seeds are traditionally brewed to prepare coffee and taken orally as a laxative, diuretic, stimulant, blood cleanser and for treating sexually transmitted infections or used as a mouthwash for toothache and inflammation of the mouth or gums. The seeds tinctured in alcohol are applied topically for arthritis and muscle pain. The leaves are typically prepared as a tea by infusion and taken orally for diarrhea, and may also be prepared as a bath for skin ailments.

Safety: The seeds and seed decoction are widely consumed and generally considered safe. One of the primary active constituents in coffee is caffeine. Potential adverse effects from excess coffee intake include diarrhea, insomnia, headache, heart palpitations, hyperacidity and stomach irritation. No data on the safety of the leaf in humans has been identified in the available literature. In animal studies, the leaf showed no evident signs of toxicity.

Contraindications: Excess caffeine consumption (including coffee) is not advised during pregnancy or lactation. Caution is advised in patients with renal dysfunction and hyperthyroidism. No data on the safety of the leaves in pregnancy, lactation or small children has been identified in the available literature.

Drug Interactions: Coffee may interfere with drug resorption. The following medications may inhibit caffeine metabolism or clearance: oral contraceptives, cimetidine, furafylline, verapamil, disulfiram, fluconoazole, mexiletine, phenylpropanolamine, numerous quinolone antibiotics (i.e. enoxacin, pipemidic acid, ciprofloxacin, norfloxacin), idrocilamide and methoxsalen.

Clinical Data: Caffeine has been investigated in human clinical trials for its cognitive enhancement effects, and coffee has been studied as a colonic stimulant and common cold treatment.

Ayurvedic: Amarvalli, Aakaashbel. (Cuscuta reflexa is also known as Amarvalli.)

Siddha/Tamil: Erumaikkottan.

Action: Astringent, diuretic (given in dropsy and anasarca, also in biliousness, chronic dysentery, haemoptysis and for supressing lactation after still-birth); piscicidal and insecticidal (used as a hair-wash for killing vermin).

The plant contains aporphine alkaloids. calcium, phosphorus, thiamine, riboflavin and niacin; also tocopherols. Nuts, crushed with vinegar and barley flour, are used against indurations of breast. The extract of nuts exhibits possibility of its use as a platelet inhibitor in thrombosis and atherosclerosis. Leaves are inhibitors of pectinolytic enzymes.

Cundeamor

Bitter melon (Momordica charantia).

Ayurvedic: Dudhi, Dudhikaa, Naagaarjuni, Vikshirini.

Siddha/Tamil: Amman pachharisi.

Action: Pectoral, antiasthmatic, antispasmodic. Used for asthma, laryngitis, chronic nasal and bronchial catarrh; diarrhoea, dysentery, intestinal parasitosis Also used in postnatal complaints, failure of lactation. Latex— vermifuge. Used in diseases of urinogenitory tract.

The herb contains several terpenes, anthocyanins, alcohols and steroids. Aerial parts also gave shikimic acid, choline, L-inositol and free sugars.

Antiasthmatic activity is attributed to choline and shikimic acid. Shikimic acid and choline showed relaxant and contracting properties on guinea-pig ileum.

The aqueous extract of the herb exhibited sedative, anxiolytic, analgesic, antipyretic and anti-inflammatory activities; exerted an inhibitory effect on platelet aggregation.

Quercitrin is reported to be responsible for antidiarrhoeal activity.

Methanolic extract of the leaves exhibits antibacterial and antifungal activities.

Diet

The mixture of food and drink consumed by an individual. Variations in morbidity and mortality between population groups are believed to be due, in part, to di?erences in diet. A balanced diet was traditionally viewed as one which provided at least the minimum requirement of energy, protein, vitamins and minerals needed by the body. However, since nutritional de?ciencies are no longer a major problem in developed countries, it seems more appropriate to consider a ‘healthy’ diet as being one which provides all essential nutrients in su?cient quantities to prevent de?ciencies but which also avoids health problems associated with nutrient excesses.

Major diet-related health problems in prosperous communities tend to be the result of dietary excesses, whereas in underdeveloped, poor communities, problems associated with dietary de?ciencies predominate. Excessive intakes of dietary energy, saturated fats, sugar, salt and alcohol, together with an inadequate intake of dietary ?bre, have been linked to the high prevalence of OBESITY, cardiovascular disease, dental caries, HYPERTENSION, gall-stones (see GALL-BLADDER, DISEASES OF), non-insulindependent DIABETES MELLITUS and certain cancers (e.g. of the breast, endometrium, intestine and stomach) seen in developed nations. Health-promotion strategies in these countries generally advocate a reduction in the intake of fat, particularly saturated fat, and salt, the avoidance of excessive intakes of alcohol and simple sugars, an increased consumption of starch and ?bre and the avoidance of obesity by taking appropriate physical exercise. A maximum level of dietary cholesterol is sometimes speci?ed.

Undernutrition, including protein-energy malnutrition and speci?c vitamin and mineral de?ciencies, is an important cause of poor health in underdeveloped countries. Priorities here centre on ensuring that the diet provides enough nutrients to maintain health.

In healthy people, dietary requirements depend on age, sex and level of physical activity. Pregnancy and lactation further alter requirements. The presence of infections, fever, burns, fractures and surgery all increase dietary energy and protein requirements and can precipitate undernutrition in previously well-nourished people.

In addition to disease prevention, diet has a role in the treatment of certain clinical disorders, for example, obesity, diabetes mellitus, HYPERLIPIDAEMIA, inborn errors of metabolism, food intolerances and hepatic and renal diseases. Therapeutic diets increase or restrict the amount and/or change the type of fat, carbohydrate, protein, ?bre, vitamins, minerals and/or water in the diet according to clinical indications. Additionally, the consistency of the food eaten may need to be altered. A commercially available or ‘homemade’ liquid diet can be used to provide all or some of a patient’s nutritional needs if necessary. Although the enteral (by mouth) route is the preferred route for feeding and can be used for most patients, parenteral or intravenous feeding is occasionally required in a minority of patients whose gastrointestinal tract is unavailable or unreliable over a period of time.

Infant Feeding

The newborn infant may be fed naturally from the breast, or arti?cially from a bottle.

Breast feeding Unless there is a genuine contraindication, every baby should be breast fed. The nutritional components of human milk are in the ideal proportions to promote the healthy growth of the human newborn. The mother’s milk, especially colostrum (the ?uid secreted before full lactation is established) contains immune cells and antibodies that increase the baby’s resistance to infection. From the mother’s point of view, breast feeding helps the womb to return to its normal size and helps her to lose excess body fat gained during pregnancy. Most importantly, breast feeding promotes intimate contact between mother and baby. A ?nal point to be borne in mind, however, is that drugs taken by a mother can be excreted in her milk. These include antibiotics, sedatives, tranquillisers, alcohol, nicotine and high-dose steroids or vitamins. Fortunately this is rarely a cause of trouble. (See also main entry on BREAST FEEDING.)

Arti?cial feeding Unmodi?ed cows’ milk is not a satisfactory food for the human newborn and may cause dangerous metabolic imbalance. If breast feeding is not feasible, one of the many commerciallly available formula milks should be used. Most of these are made from cows’ milk which has been modi?ed to re?ect the composition of human milk as closely as possible. For the rare infant who develops cows’-milk-protein intolerance, a milk based on soya-bean protein is indicated.

Feeding and weight gain The main guide as to whether an infant is being adequately fed is the weight. During the ?rst days of life a healthy infant loses weight, but should by the end of the second week return to birth weight. From then on, weight gain should be approximately 6oz. (170g) each week.

The timing of feeds reffects social convention rather than natural feeding patterns. Among the most primitive hunter-gatherer tribes of South America, babies are carried next to the breast and allowed to suckle at will. Fortunately for developed society, however, babies can be conditioned to intermittent feedings.

As the timing of breast feeding is ?exible – little or no preparation time being required – mothers can choose to feed their babies on demand. Far from spoiling the baby, demand feeding is likely to lead to a contented infant, the only necessary caution being that a crying baby is not always a hungry baby.

In general, a newborn will require feeding every two to four hours and, if well, is unlikely to sleep for more than six hours. After the ?rst months, a few lucky parents will ?nd their infant sleeping through the night.

Weaning Weaning on to solid foods is again a matter of individuality. Most babies will become dissatis?ed with a milk-only diet at around six months and develop enthusiasm for cereal-based weaning foods. Also at about this time they enjoy holding objects and transferring them to their mouths – the mouth being an important sense organ in infants. It is logical to include food items that they can hold, as this clearly brings the baby pleasure at this time. Introduction of solids before the age of four months is unusual and best avoided. The usual reason given for early weaning is that the baby appears hungry, but this is unlikely to be the case; crying due to COLIC, for example, is more probable. Some mothers take the baby’s desire to suck – say, on their ?nger – as a sign of hunger when this is, in fact, re?ex activity.

Delaying the start of weaning beyond nine months is nutritionally undesirable. As weaning progresses, the infant’s diet requires less milk. Once established on a varied solid diet, breast and formula milks can be safely replaced with cows’ milk. There is, however, no nutritional contraindication to continued breast feeding until the mother wishes to stop.

It is during weaning that infants realise they can arouse extreme maternal anxiety by refusing to eat. This can lead to force-feeding and battles of will which may culminate in a breakdown of the mother-child relationship. To avoid this, parents must resist the temptation to coax the child to eat. If the child refuses solid food, the meal should be taken away with a minimum of fuss. Children’s appetites re?ect their individual genetic structure and a well child will eat enough to grow and maintain satisfactory weight gain. If a child is not eating properly, weight gain will be inadequate over a prolonged period and an underlying illness is the most likely cause. Indeed, failure to thrive is the paediatrician’s best clue to chronic illness.

Advice on feeding Many sources of con?icting advice are available to new parents. It is impossible to satisfy everyone, and ultimately it is the well-being of the mother and infant and the closeness of their relationship that matter. In general, mothers should be wary of rigid advice. An experienced midwife, health visitor or well-baby-clinic nursing sister are among the most reliable sources of information.

Pituitary Gland

Also known as the pituitary body and the hypophysis, this is an ovoid structure, weighing around 0·5 gram in the adult. It is attached to the base of the BRAIN, and lies in the depression in the base of the skull known as the sella turcica. The anterior part is called the adenohypophysis and the posterior part the neurohy-P pophysis. The gland is connected to the HYPOTHALAMUS of the brain by a stalk known as the hypophyseal or pituitary stalk.

The pituitary gland is the most important ductless, or endocrine, gland in the body. (See

ENDOCRINE GLANDS.) It exerts overall control of the endocrine system through the media of a series of hormones which it produces. The adenohypophysis produces trophic hormones (that is, they work by stimulating or inhibiting other endocrine glands) and have therefore been given names ending with ‘trophic’ or ‘trophin’. The thyrotrophic hormone, or thyroid-stimulating hormone (TSH), exerts a powerful in?uence over the activity of the THYROID GLAND. The ADRENOCORTICOTROPHIC HORMONE (ACTH) stimulates the cortex of the adrenal glands. GROWTH HORMONE, also known as somatotrophin (SMH), controls the growth of the body. There are also two gonadotrophic hormones which play a vital part in the control of the gonads: these are the follicle-stimulating hormone (FSH), and the luteinising hormone (LH) which is also known as the interstitial-cell-stimulating hormone (ICSH) – see GONADOTROPHINS. The lactogenic hormone, also known as prolactin, mammotrophin and luteotrophin, induces lactation.

The neurohypophysis produces two hormones. One is oxytocin, which is widely used because of its stimulating e?ect on contraction of the UTERUS. Breast Tenderness

Soreness or tenderness of the breasts, often with a feeling of fullness. Breast tenderness is an extremely common problem. In most women it is cyclical, varying in severity in response to the hormonal changes of the menstrual cycle. It is usually most severe before a period (see premenstrual syndrome). It tends to affect both breasts and may be aggravated by stress or caffeine in drinks. Breast tenderness can also be noncyclical and may be caused by muscle strain or mastitis. During lactation, it may be due to engorgement with milk. Rarely, tenderness may be due to a breast cyst or breast cancer. However, examination by a doctor will exclude any underlying problems. Women with large breasts are more likely to suffer from both cyclical and noncyclical breast tenderness.

Cyclical tenderness may be relieved by reduced caffeine intake, relaxation

exercises for stress, a well-fitting bra, or weight loss to reduce breast size.

Energy Requirements

The amount of energy that is needed by a person for cell metabolism, muscular activity, and growth. This energy is provided by the breakdown of carbohydrates, fats, and proteins supplied by food in the diet and by stored nutrients in the liver, muscles, and adipose tissue.

Energy is needed to maintain the heartbeat, lung function, and constant body temperature. The rate at which these processes use energy is called the basal metabolic rate (BMR). Any form of movement increases energy expenditure above the BMR. A person’s energy requirement increases during periods of growth and during pregnancy and lactation. When more energy is ingested as food than is used, the surplus is stored and there is usually a gain in weight. Galactorrhoea

Spontaneous, persistent production of milk by a woman who is not pregnant or lactating (see lactation), or, very rarely, by a man.

Lactation is initiated by a rise in the level of prolactin, a hormone produced by the pituitary gland. Galactorrhoea is caused by excessive secretion of prolactin due to a pituitary tumour or otherendocrine disease, such as hypothyroidism. Some antipsychotic drugs may also cause excessive secretion. Treatment with bromocriptine suppresses prolactin production, but the underlying cause may also need treatment. galactosaemia A rare, inherited condition in which the body is unable to convert the sugar galactose into glucose due to the absence of a liver enzyme. It causes no symptoms at birth, but jaundice, diarrhoea, and vomiting soon develop and the baby fails to gain weight. Untreated, the condition results in liver disease, cataract, and learning difficulties. The diagnosis is confirmed by urine and blood tests. The major source of galactose is the milk sugar lactose. Lactosefree milk must be used throughout life. gallbladder A small, pear-shaped sac situated under the liver that stores bile. Prolactin

Prolactin is the pituitary hormone (see PITUITARY GLAND) which initiates lactation. The development of the breasts during pregnancy is ascribed to the action of OESTROGENS; prolactin starts them secreting. If lactation does not occur or fails, it may be started by injection of prolactin.

The secretion of prolactin is normally kept under tonic inhibition by the secretion of DOPAMINE which inhibits prolactin. This is formed in the HYPOTHALAMUS and secreted into the portal capillaries of the pituitary stalk to reach the anterior pituitary cells. Drugs that deplete the brain stores of dopamine or antagonise dopamine at receptor level will cause HYPERPROLACTINAEMIA and hence the secretion of milk from the breast and AMENORRHOEA. METHYLDOPA and RESERPINE deplete brain stores of dopamine and the PHENOTHIAZINES act as dopamine antagonists at receptor level. Other causes of excess secretion of prolactin are pituitary tumours, which may be minute and are then called microadenomas, or may actually enlarge the pituitary fossa and are then called macroadenomas. The most common cause of hyperprolactinaemia is a pituitary tumour. The patient may present with infertility – because patients with hyperprolactinaemia do not ovulate – or with amenorrhea and even GALACTORRHOEA.

BROMOCRIPTINE is a dopamine agonist. Treatment with bromocriptine will therefore control hyperprolactinaemia, restoring normal menstruation and ovulation and suppressing galactorrhoea. If the cause of hyperprolactinaemia is an adenomatous growth in the pituitary gland, surgical treatment should be considered.... prolactin

Aromatherapy

The external use of essential oils from seeds, resins, herbs, barks and spices for relaxant purposes.

Plant essences give plants their scent and were known to the ancient civilisations of Egypt and Greece as the ‘vital force’ or spirit of the plant. They were used for inhalation, rubbing on the skin or as a healthful addition to baths and foot-baths. The art is complementary to phytotherapy, acupuncture and other systems of alternative medicine.

The aromatherapist uses oils individually or in blends of different oils. The natural concentrated oil is usually diluted by adding a vegetable oil before direct application to the skin. A massage oil usually comprises 6 drops essential oil to 10ml (2 teaspoons) carrier oil – Almond, Peanut or other vegetable oil.

The skin is known to be an integral part of the immune system. T-cells are scattered throughout, primarily in the epidermis or outer layer. It has been demonstrated that oils rubbed on the skin are readily absorbed and borne to distant organs in the body via the bloodstream to soothe, relax and heal. Some oils should not be used during pregnancy or lactation.

An oil may be used as a natural perfume. As a bath oil, 5-6 drops of a favourite oil may be added to bathwater. Oils freshen a room; stimulate or relax as desired when added to water on a warm radiator. Oils are never used on the skin undiluted.

The aromatherapist never uses essential oils internally. Other carrier oils may be used: Sesame seed, Sunflower seed, Apricot kernel and Wheatgerm. Usual methods of applying essential oils: massage, inhalation and baths. When adding oils to baths water should not be too hot which causes oils to evaporate.

Remedies absorbed into the body via the skin avoid metabolism by the liver as when taken by mouth.

When the therapy was used in a geriatric ward in Oxford drug expenditure on laxatives and night sedatives fell. It was reported to have given profoundly deaf patients, many of whom had multiple sensory deficits, tranquillity. The results of a randomised trial in patients on an intensive care unit showed significantly greater psychological improvement (as demonstrated with anxiety and mood rating scales) in those given aromatherapy (1 per cent Lavender and Grapeseed oil) over those massaged with Grapeseed oil only or those prescribed rest alone. Bearberry

Arctostaphylos uva-ursi, Spreng. French: Busserole. German: Gemeine Ba?rrentraube. Italian: Uva d’orso. Dried leaves.

Contains hydroquinones, iridoids, flavonoids. Keynote: highly acid urine.

Action. Diuretic, urinary antiseptic, astringent, haemostatic, oxytocic.

Uses: smarting cystitis, painful micturition, urethritis, blood in the urine, urinary retention, oedema of legs or face, bed-wetting, diarrhoea, dysentery, profuse menstruation, leucorrhoea (chronic).

Combinations. With Dandelion root for dropsy. With Broom, Buchu and Clivers for inflammation of urinary tract and bladder. With Couch Grass as a urinary antiseptic.

Soothing combination for kidney relief and renal backache: Bearberry 15 per cent, Couchgrass 15 per cent, Wild Carrot 15 per cent, Buchu 10 per cent, Alfalfa 45 per cent. Tea: 1 heaped teaspoon to each cup boiling water. Infuse 10 minutes, 1 cup twice daily.

Preparations: Thrice daily.

Action: relaxing nervine, sedative, spasmolytic, vaso-dilator, anti-arthritic, anti-inflammatory, anti- rheumatic, anti-cough, regulates autonomic system, emmenagogue, natural source of salicylic acid which has an aspirin-like effect. The agent works powerfully upon the female reproductive organs. Analgesic (mild).

Constituents: triterpine glycosides.

Uses: Cramps, sciatica, low back pain, facial and intercostal neuralgia, stiff neck, aches after strenuous exercise. Painful menstruation and menopausal symptoms, breast pains, threatened abortion, migraine of hormonal origin and pain in the ovaries. Tinnitus. Oestrogen-deficiency. Scarlet fever. Fatty heart.

Combinations: With Black Cohosh for muscular pain. Also for tenderness of the spinal vertebre (an important indication). With Poke root for inflammation of the breast or testicles.

Preparations: Owing to difficulty of the layman to dispense accurately dosage of powder or decoction, use is best confined to liquid extract or tincture; small doses frequently repeated; large doses avoided. Liquid Extract: 10 drops in 4oz water; dose 1 teaspoon every half hour.

Preparations: Thrice daily. Persistence with low doses is more favourable than larger, over short periods. Some herbalists have observed more favourable results from use of the decoction.

Liquid Extract. BHC Vol 1 (root). 1:1, 25 per cent ethanol. Dose: 2-6ml.

Action: anticatarrhal, relaxing expectorant, demulcent bitter, diuretic. Immune stimulant. Antispasmodic. Anti-inflammatory. Antitussive.

Uses: Relief of dry unproductive irritative cough, smoker’s cough, whooping cough, bronchial asthma. Dr J. Cullen found a strong decoction of the leaves beneficial for tuberculosis. Dr E. Percival found it useful in hectic diarrhoea. Has been used with limited success in silicosis and pneumoconiosis. Rubbed herb once used as smoking mixture for bronchial conditions.

Combination. Dried herb: equal parts: Coltsfoot, Thyme, White Horehound. Mix. 1oz to 1 pint water gently simmered 5 minutes. Dose: half-1 cup thrice daily.

Preparations: Thrice daily.

Decoction. Half a teaspoon to each cup water gently simmered 5 minutes: dose – half-1 cup.

Liquid Extract. 1-2ml.

Tincture BHP (1983): 1:5 in 45 per cent alcohol: dose 2-8ml.

Grows freely in Russia where it is cultivated and the fresh root preserved in strong vodka for chest and stomach complaints.

Constituents: sesquiterpene lactones, inulin, resin.

Action: antispasmodic, alterative, stimulating expectorant, diaphoretic, antiseptic, stomachic, anticatarrhal.

“Performs a double action (1) as a bacteriostatic in chronic bronchitis and (2) its biochemical action in alleviating rheumatism.” (Hamdard Foundation, Pakistan).

Combines well with Yarrow and Marshmallow. Popular combination. Decoction: mix equal parts; Yarrow, Marshmallow root and Elecampane root. 1oz (30g) in 1 pint (500ml) water simmered 20 minutes. Dose: half-1 cup, 2-3 times daily. In the absence of any one ingredient, substitute Wild Cherry bark.

Preparations: Thrice daily.

Powder: quarter to half a teaspoon.

Decoction, root. Quarter to half a teaspoon to each cup of water; simmer gently 10 minutes in a covered vessel. Dose: 1 cup.

Goldenseal

Yellow root. Eye Balm. Hydrastis canadensis L. Dried rhizome and roots. One of phytotherapy’s most effective agents. Versatile, with a wide sphere of influence.

Constituents: berberine, hydrastine, canadine, resin.

Action: alterative, choleretic, antiseptic, anti-inflammatory, anti-microbial, bacteriostatic against staphyococcus. (Complementary Medical Research Vol 2, No 2, p.139) Bitter, diuretic, haemostatic, laxative, oxytocic, powerful stomach and liver tonic, detoxifier. Increases blood supply to the spleen. Uses. Mucous membranes generally. Ulceration of mouth, throat, intestines. Heartburn, chronic dyspepsia, gastric and duodenal ulcer, diverticulosis, ulcerative colitis, liver damage. To assist function of old age. Drying to mucous surfaces and therefore indicated in all forms of catarrh (respiratory, vaginal etc). Proteinuria. Painful, excessive menstruation and bleeding from the womb for which the addition of Beth root (equal parts) enhances action. Itching of anus and genitals. Liquid Extract, BHC Vol 1. 1:1 in 60 per cent ethanol; 0.3-1ml, (5-15 drops).

Tincture, BHC Vol 1. 1:10, 60 per cent ethanol; 2-4ml, (15-60 drops).

Formula. Popular. All BHP (1983) standard powders:– Marshmallow root 100mg; Goldenseal 10mg; Cranesbill 30mg; Dandelion root 60mg. Traditional for the relief of indigestion, heartburn, flatulence, nausea and gastric irritation.

Lotion. Equal parts, Tincture Goldenseal and glycerine. For painting mouth, throat and lesions elsewhere. Goldenseal solution. 250mg powder shaken in 3oz Rosewater or Witch Hazel: filter. 5-10 drops in eyebath half-filled with water; douche 3 or more times daily.

Goldenseal ointment. 1 teaspoon (5ml) tincture in 1oz Vaseline; dissolve in gentle heat.

Infanticide

n. (in England and Wales) under the terms of the Infanticide Act 1938, the killing of an infant by the natural mother within 12 months of birth. Where it can be shown that the woman’s balance of mind was disturbed due to childbirth and/or lactation she would not normally be charged with murder but with infanticide, which carries a much lighter sentence. As a result, it is uncommon for a mother who kills her infant child to receive a custodial sentence. Fenugreek

Trigonella foenum-graecum

FAMILY: Fabaceae

SYNONYMS: Greek hay, methi, alholva, bird’s foot, chandrika, fenigreek, Greek clover.

GENERAL DESCRIPTION: An annual plant reaching a height of slightly more than half a metre with trifoliate, bright green leaves. It blossoms during the summer season and produces beautiful small white flowers. The leaves’ fragrance slightly resembles lovage and the brownish-yellow seeds are a distinctive cuboid shape.

DISTRIBUTION: An aromatic herb native to southern Europe and Asia and one of the oldest cultivated medicinal plants. It is widely grown today in the Mediterranean countries, Argentina, India, North Africa and the United States. Major commercial fenugreek-producing countries are Afghanistan, Pakistan, Iran, Nepal, Bangladesh, Argentina, Egypt, France, Spain, Turkey and Morocco, although the largest producer is India. When harvested as micro-greens, fenugreek is known as ‘samudra methi’ in Mumbai.

OTHER SPECIES: The cultivated fenugreek is thought to have derived from a wild species, native to India.

HERBAL/FOLK TRADITION: Fenugreek is an ancient spice, although currently not much used in the West. While the seeds and leaves have primarily been used as a culinary spice, it was also employed in classical times as livestock feed, hence its common name, ‘Greek hay’. It was used by the ancient Egyptians to combat fever and in the embalming process. It has a long history of use as a medicinal plant in India and throughout Europe: it is still used in Ayurvedic medicine, where it is known as ‘methi’. Fenugreek seeds have been found to contain diosgenin, which is a compound that has properties similar to oestrogen. For this reason, fenugreek has a long history of use as a breast enlarger and to promote the growth of new breast cells. Due to its oestrogen-like properties, fenugreek has been found to help increase sexual libido and lessen the effect of the hot flushes and mood fluctuations that are common symptoms of the menopause and PMS. The plant can also be employed to promote lactation, induce labour and as an aphrodisiac.

In Indian and Chinese medicine, it is traditionally used to treat arthritis, asthma, bronchitis, improve digestion, maintain a healthy metabolism, increase male potency, cure skin problems (wounds, rashes and boils), sore throat and heartburn. Recent studies have shown that fenugreek helps lower blood glucose and cholesterol levels, and may be an effective treatment for both type 1 and 2 diabetes; it is also being studied for its cardiovascular benefits. Topically, the gelatinous texture of fenugreek seed is used for soothing eczema or other skin complaints; it is also applied as a warm poultice to relieve muscle aches and gout. Fenugreek is also nutritious as it is a valuable source of vitamins and minerals, particularly vitamins A, C and D as well as iron and calcium. Fenugreek seeds are rich in protein and carbohydrates and the pressed oil is strangely similar in chemical composition to cod liver oil. It also contains choline and lecithin, both substances needed by the liver for cholesterol metabolism. Fenugreek cleanses, stimulates and helps remove waste products from the body: it stimulates the lymphatic system and aids the kidneys and liver in detoxifying the entire system. The oil contains powerful antioxidants that help slow the ageing process.

EXTRACTION: Essential oil by steam distillation from the seeds. A fixed oil is also pressed from the seeds.

CHARACTERISTICS: Pale yellow to light brown liquid with a bitter, spicy scent. It blends well with sandalwood, clove, cinnamon, cardamon and other spice oils.

PRINCIPAL CONSTITUENTS: The essential oil is rich in phytic acid, saponins and trigonelline.

SAFETY DATA: The oil may cause skin irritation: dilute well before use. Avoid use during pregnancy, since it has the potential to induce labour.
