Tonsillitis: From 3 Different Sources


Inflammation of the tonsils as a result of infection. Tonsillitis mainly occurs in children under age 9. Sometimes the tonsils become repeatedly infected by the microorganisms they are supposed to protect against. The main symptoms are a sore throat and difficulty in swallowing. The throat is visibly inflamed. Other common symptoms are fever, headache, earache, enlarged and tender lymph nodes in the neck, and bad breath. Occasionally, there may be temporary deafness or quinsy (an abscess around the tonsil).

Tonsillitis is treated with plenty of fluids and an analgesic drug such as paracetamol; in some cases antibiotic drugs may also be prescribed.

Tonsillitis is the in?ammation of the TONSILS. The disorder may be the precurosor of a virus-induced infection of the upper respiratory tract such as the COMMON COLD, INFLUENZA or infectious MONONUCLEOSIS, in which case the in?ammation usually subsides as other symptoms develop. Such virus-induced tonsillitis does not respond to treatment with antibiotics. This section describes tonsillitis caused by bacterial infection.

Acute tonsillitis The infection is never entirely con?ned to the tonsils; there is always some involvement of the surrounding throat or pharynx. The converse is true that in many cases of ‘sore throat’, the tonsils are involved in the generalised in?ammation of the throat.

Causes Most commonly caused by the ?haemolytic STREPTOCOCCUS, its incidence is highest in the winter months. In the developing world it may be the presenting feature of DIPHTHERIA, a disease now virtually non-existant in the West since the introduction of IMMUNISATION.

Symptoms The onset is usually fairly sudden with pain on swallowing, fever and malaise. On examination, the tonsils are engorged and covered with a whitish discharge (PUS). This may occur at scattered areas over the tonsillar crypts (follicular tonsillitis), or it may be more extensive. The glands under the jaw are enlarged and tender, and there may be pain in the ear on the affected side: although usually referred pain, this may indicate spread of the infection up the Eustachian tube to the ear, particularly in children. Occasionally an ABSCESS, or quinsy, develops around the affected tonsil. Due to a collection of pus, it usually comes on four to ?ve days after the onset of the disease, and requires specialist surgical treatment.

Treatment Most cases need no treatment. Therefore, it is advisable to take a throat swab to assess the nature of any bacterial treatment before starting treatment. Penicillin or erythromycin are the drugs of choice where betahaemolytic streptococci are isolated, together with paracetamol or aspirin, and plenty of ?uids. Removal of tonsils is indicated: when the tonsils and adenoids are permanently so enlarged as to interfere with breathing (in such cases the adenoids are removed as well as the tonsils); when the individual is subject to recurrent attacks of acute tonsillitis which are causing signi?cant debility, absence from school or work on a regular basis (more than four times a year); when there is evidence of a tumour of the tonsil. Recurrent sore throat is not an indication for removing tonsils.

Sore Throat

A raw sensation at the back of the throat. A common symptom, the cause is usually PHARYNGITIS, sometimes TONSILLITIS. It is often the presenting symptom of colds, INFLUENZA, LARYNGITIS and infectious MONONUCLEOSIS. Sore throats caused by streptococcal infection (see STREPTOCOCCUS) should be treated with antibiotics, as should other bacteria-initiated sore throats; otherwise, symptomatic treatment with analgesics and antiseptic gargles is suf?cient for this usually self-limiting condition.... sore throat

The term applied to a collection of pus or an ABSCESS which occurs complicating an attack of TONSILLITIS. The collection of pus forms between the tonsil and the superior constrictor muscle of the pharynx. Action: Oleo-gum-resin—em- menagogue (used for irregular menstruation and painful periods), anti-inflammatory (on pharyngitis and gingivitis), antiseptic, bacteriostatic, antiviral, astringent, stimulant, expectorant, stomachic, carminative (in dyspepsia), a leuco- cytogenic agent (increases number of white cells in the blood). Used externally for treating acne, boils and pressure sores, internally as a blood purifier.

Key application: In topical treatment of mild inflammations of the oral and pharyngeal mucosa. (German Commission E.) As a gargle or mouth rinse for the treatment of aphthous ulcers, tonsillitis, common cold and gingivitis. (The British Herbal Pharmacopoeia, ESCOP.)

The gum (30-60%) contains acidic polysaccharides, volatile oil (2-10%) including other constituents, heer- abolene, eugenol, furanosequiterpenes and monoterpenes.

Myrrh is taken as a powder or a tincture, rather than as an infusion; used generally externally or as a gargle.

Aqueous suspension of the gum resin decreased ethanol-induced and indomethacin-induced ulcer in rats. (JEthnopharmacol, 1997, Jan 55(2), 141150.)

Synonym: Madhuca longifolia (Linn.) Macbride.

Family: Sapotaceae.

Habitat: South India; common in the monsoon forests of Western Ghats.

English: Mowra Butter tree, South Indian Mahua.

Siddha/Tamil: Illupei, Elupa, Naatu, Iluppei, Iruppei.

Action: Flowers—laxative, bechic (used in coughs, colds and bronchitis), stimulant and nervine tonic. Seed oil—galactogenic, anticephalalgic, laxative in cases of habitual constipation and piles; used externally in rheumatism and skin affections. Bark, seed oil and gum—antirheumatic.

The herb contains 17% tannins and is used for bleeding and spongy gums, tonsillitis, ulcers, rheumatism and diabetes mellitus. Roots are applied to ulcers.

Seed kernel gave protobassic acid (a sapogenol) and two major saponins— Mi-saponins A and B. Mi-saponins (bisdesmosides of protobassic acid) exhibit anti-inflammatory activity in rheumatism.

The carollas are a rich source of sugars and contain an appreciable amount of vitamins and calcium (total sugars 72.9%, calcium 140 mg/100 g). Sugars are identified as sucrose, maltose, glucose, fructose, arabinose and rham- nose. Flowers are largely used in the preparation of distilled liquors. Belamcanda Chinensis

(L.) DC.

Family: Iridaceae.

Habitat: Introduced from China; cultivated all over India, up to an altitude of 1,800 m.

Folk: Surajkaanti (Assam), Dasbaha, Dasbichandi (Bengal).

Action: Rhizomes—expectorant, deobstruent, resolvent, used in tonsillitis, chest and liver complaints (antiviral against pneumonia).

Presence of alkaloids is reported from the plant, glucoside, belamcan- din from the roots. The leaves and flowers contain a glycoflavone. The seeds tested positive for leucoantho- cyanins.... belamcanda chinensis

Ear, Diseases Of

Diseases may affect the EAR alone or as part of a more generalised condition. The disease may affect the outer, middle or inner ear or a combination of these.

Examination of the ear includes inspection of the external ear. An auriscope is used to examine the external ear canal and the ear drum. If a more detailed inspection is required, a microscope may be used to improve illumination and magni?cation.

Tuning-fork or Rinne tests are performed to identify the presence of DEAFNESS. The examiner tests whether the vibrating fork is audible at the meatus, and then the foot of the fork is placed on the mastoid bone of the ear to discover at which of the two sites the patient can hear the vibrations for the longest time. This can help to di?erentiate between conductive and nerve deafness.

Hearing tests are carried out to determine the level of hearing. An audiometer is used to deliver a series of short tones of varying frequency to the ear, either through a pair of headphones or via a sound transducer applied directly to the skull. The intensity of the sound is gradually reduced until it is no longer heard and this represents the threshold of hearing, at that frequency, through air and bone respectively. It may be necessary to play a masking noise into the opposite ear to prevent that ear from hearing the tones, enabling each ear to be tested independently.

General symptoms The following are some of the chief symptoms of ear disease: DEAFNESS (see DEAFNESS). EARACHE is most commonly due to acute in?ammation of the middle ear. Perceived pain in this region may be referred from other areas, such as the earache commonly experienced after tonsillectomy (removal of the TONSILS) or that caused by carious teeth (see TEETH, DISORDERS OF). The treatment will depend on the underlying cause. TINNITUS or ringing in the ear often accompanies deafness, but is sometimes the only symptom of ear disease. Even normal people sometimes experience tinnitus, particularly if put in soundproofed surroundings. It may be described as hissing, buzzing, the sound of the sea, or of bells. The intensity of the tinnitis usually ?uctuates, sometimes disappearing altogether. It may occur in almost any form of ear disease, but is particularly troublesome in nerve deafness due to ageing and in noise-induced deafness. The symptom seems to originate in the brain’s subcortical regions, high in the central nervous system. It may be a symptom of general diseases such as ANAEMIA, high blood pressure and arterial disease, in which cases it is often synchronous with the pulse, and may also be caused by drugs such as QUININE, salicylates (SALICYLIC ACID and its salts, for example, ASPIRIN) and certain ANTIBIOTICS. Treatment of any underlying ear disorder or systemic disease, including DEPRESSION, may reduce or even cure the tinnitis, but unfortunately in many cases the noises persist. Management involves psychological techniques and initially an explanation of the mechanism and reassurance that tinnitus does not signify brain disease, or an impending STROKE, may help the person. Tinnitus maskers – which look like hearing aids – have long been used with a suitably pitched sound helping to ‘mask’ the condition.

Diseases of the external ear

WAX (cerumen) is produced by specialised glands in the outer part of the ear canal only. Impacted wax within the ear canal can cause deafness, tinnitis and sometimes disturbance of balance. Wax can sometimes be softened with olive oil, 5-per-cent bicarbonate of soda or commercially prepared drops, and it will gradually liquefy and ‘remove itself’. If this is ineffective, syringing by a doctor or nurse will usually remove the wax but sometimes it is necessary for a specialist (otologist) to remove it manually with instruments. Syringing should not be done if perforation of the tympanic membrane (eardrum) is suspected. FOREIGN BODIES such as peas, beads or buttons may be found in the external ear canal, especially in children who have usually introduced them themselves. Live insects may also be trapped in the external canal causing intense irritation and noise, and in such cases spirit drops are ?rst instilled into the ear to kill the insect. Except in foreign bodies of vegetable origin, where swelling and pain may occur, syringing may be used to remove some foreign bodies, but often removal by a specialist using suitable instrumentation and an operating microscope is required. In children, a general anaesthetic may be needed. ACUTE OTITIS EXTERNA may be a di?use in?ammation or a boil (furuncle) occurring in the outer ear canal. The pinna is usually tender on movement (unlike acute otitis media – see below) and a discharge may be present. Initially treatment should be local, using magnesium sulphate paste or glycerine and 10-per-cent ichthaminol. Topical antibiotic drops can be used and sometimes antibiotics by mouth are necessary, especially if infection is acute. Clotrimazole drops are a useful antifungal treatment. Analgesics and locally applied warmth should relieve the pain.

CHRONIC OTITIS EXTERNA producing pain and discharge, can be caused by eczema, seborrhoeic DERMATITIS or PSORIASIS. Hair lotions and cosmetic preparations may trigger local allergic reactions in the external ear, and the chronic disorder may be the result of swimming or use of dirty towels. Careful cleaning of the ear by an ENT (Ear, Nose & Throat) surgeon and topical antibiotic or antifungal agents – along with removal of any precipitating cause – are the usual treatments. TUMOURS of the ear can arise in the skin of the auricle, often as a result of exposure to sunlight, and can be benign or malignant. Within the ear canal itself, the commonest tumours are benign outgrowths from the surrounding bone, said to occur in swimmers as a result of repeated exposure to cold water. Polyps may result from chronic infection of the ear canal and drum, particularly in the presence of a perforation. These polyps are soft and may be large enough to ?ll the ear canal, but may shrink considerably after treatment of the associated infection.

Diseases of the middle ear

OTITIS MEDIA or infection of the middle ear, usually occurs as a result of infection spreading up the Eustachian tubes from the nose, throat or sinuses. It may follow a cold, tonsillitis or sinusitis, and may also be caused by swimming and diving where water and infected secretions are forced up the Eustachian tube into the middle ear. Primarily it is a disease of children, with as many as 1.5 million cases occurring in Britain every year. Pain may be intense and throbbing or sharp in character. The condition is accompanied by deafness, fever and often TINNITUS.

In infants, crying may be the only sign that something is wrong – though this is usually accompanied by some localising manifestation such as rubbing or pulling at the ear. Examination of the ear usually reveals redness, and sometimes bulging, of the ear drum. In the early stages there is no discharge, but in the later stages there may be a discharge from perforation of the ear drum as a result of the pressure created in the middle ear by the accumulated pus. This is usually accompanied by an immediate reduction in pain.

Treatment consists of the immediate administration of an antibiotic, usually one of the penicillins (e.g. amoxicillin). In the majority of cases no further treatment is required, but if this does not quickly bring relief then it may be necessary to perform a myringotomy, or incision of the ear drum, to drain pus from the middle ear. When otitis media is treated immediately with su?cient dosage of the appropriate antibiotic, the chances of any permanent damage to the ear or to hearing are reduced to a negligible degree, as is the risk of any complications such as mastoiditis (discussed later in this section). CHRONIC OTITIS MEDIA WITH EFFUSION or glue ear, is the most common in?ammatory condition of the middle ear in children, to the extent that one in four children in the UK entering school has had an episode of ‘glue ear’. It is characterised by a persistent sticky ?uid in the middle ear (hence the name); this causes a conductive-type deafness. It may be associated with enlarged adenoids (see NOSE, DISORDERS OF) which impair the function of the Eustachian tube. If the hearing impairment is persistent and causes problems, drainage of the ?uid, along with antibiotic treatment, may be needed – possibly in conjunction with removal of the adenoids. The insertion of grommets (ventilation tubes) was for a time standard treatment, but while hearing is often restored, there may be no long-term gain and even a risk of damage to the tympanic membrane, so the operation is less popular than it was a decade or so ago. MASTOIDITIS is a serious complication of in?ammation of the middle ear, the incidence of which has been dramatically reduced by the introduction of antibiotics. In?ammation in this cavity usually arises by direct spread of acute or chronic in?ammation from the middle ear. The signs of this condition include swelling and tenderness of the skin behind the ear, redness and swelling inside the ear, pain in the side of the head, high fever, and a discharge from the ear. The management of this condition in the ?rst instance is with antibiotics, usually given intravenously; however, if the condition fails to improve, surgical treatment is necessary. This involves draining any pus from the middle ear and mastoid, and removing diseased lining and bone from the mastoid.

Diseases of the inner ear

MENIÈRE’S DISEASE is a common idiopathic disorder of ENDOLYMPH control in the semicircular canals (see EAR), characterised by the triad of episodic VERTIGO with deafness and tinnitus. The cause is unknown and usually one ear only is affected at ?rst, but eventually the opposite ear is affected in approximately 50 per cent of cases. The onset of dizziness is often sudden and lasts for up to 24 hours. The hearing loss is temporary in the early stages, but with each attack there may be a progressive nerve deafness. Nausea and vomiting often occur. Treatment during the attacks includes rest and drugs to control sickness. Vasodilator drugs such as betahistine hydrochloride may be helpful. Surgical treatment is sometimes required if crippling attacks of dizziness persist despite these measures. OTOSCLEROSIS A disorder of the middle ear that results in progressive deafness. Often running in families, otosclerosis affects about one person in 200; it customarily occurs early in adult life. An overgrowth of bone ?xes the stapes (the innermost bone of the middle ear) and stops sound vibrations from being transmitted to the inner ear. The result is conductive deafness. The disorder usually affects both ears. Those affected tend to talk quietly and deafness increases over a 10–15 year period. Tinnitus often occurs, and occasionally vertigo.

Abnormal hearing tests point to the diagnosis; the deafness may be partially overcome with a hearing aid but surgery is eventually needed. This involves replacing the stapes bone with a synthetic substitute (stapedectomy). (See also OTIC BAROTRAUMA.)... ear, diseases of

Febrile Convulsion

Convulsion occurring in a child aged six months to six years with a high temperature in which the limbs twitch; he or she may lose consciousness. The condition is common, with one child in 20 suffering from it. It is a result of immature homeostatic control (see HOMEOSTASIS) and is not usually serious, occurring generally during an infection such as measles or tonsillitis. The brain and nervous system are normal in most cases. Treatment is tepid sponging and attention to the underlying cause, with the child placed in the recovery position. It is important to rule out more serious illness, such as MENINGITIS, if the child seems particularly ill.... febrile convulsion

Gargling is a process by which various substances in solution are brought into contact with the throat without being swallowed. The watery solutions used for the purpose are called gargles. Importance: Indian bdellium is a small, armed, deciduous tree from the bark of which gets an aromatic gum resin, the ‘Guggul’ of commerce. It is a versatile indigenous drug claimed by ayurvedists to be highly effective in the treatment of rheumatism, obesity, neurological and urinary disorders, tonsillitis, arthritis and a few other diseases. The fumes from burning guggul are recommended in hay- fever, chronic bronchitis and phytises.

The price of guggulu gum has increased ten fold in ten years or so, indicating the increase in its use as well as decrease in natural plant stand. It has been listed as a threatened plant by Botanical Survey of India (Dalal, 1995) and is included in the Red Data Book (IUCN) and over exploited species in the country (Billare,1989).

Distribution: The center of origin of Commiphora spp. is believed to be Africa and Asia. It is a widely adapted plant well distributed in arid regions of Africa (Somalia, Kenya and Ethiopia in north east and Madagascar, Zimbabwe, Botswana, Zaire in south west Africa), Arabian peninsula (Yemen, Saudi Arabia and Oman). Different species of Commiphora are distributed in Rajasthan, Gujarat, Maharashtra and Karnataka states of India and Sind and Baluchistan provinces of Pakistan (Tajuddin et al, 1994). In India, the main commercial source of gum guggul is Rajasthan and Gujarat.

Botany: The genus Commiphora of family Burseraceae comprises about 185 species. Most of them occur in Africa, Saudi Arabia and adjoining countries. In India only four species have been reported. They are C. mukul(Hook. ex Stocks) Engl. syn. Balsamodendron mukul (Hook. ex Stocks), C. wightii (Arnott) Bhandari, C.stocksiana Engl., C. berryi and C.agallocha Engl.

In early studies about the flora of India, the ‘guggul’ plant was known as Commiphora mukul(Hook. ex Stocks) Engl. or Balsamodendron mukul (Hook. ex Stocks). It was renamed as C. roxburghii by Santapau in 1962. According to Bhandari the correct Latin name of the species is C. wightii(Arnott) Bhandari, since the specific name ‘wightii’ was published in 1839, prior to ‘roxburghi’ in 1848 (Dalal and Patel, 1995).

C. mukul is a small tree upto 3-4m height with spinescent branching. Stem is brownish or pale yellow with ash colored bark peeling off in flakes. Young parts are glandular and pubescent. Leaves are alternate, 1-3 foliate, obovate, leathery and serrate (sometimes only towards the apex). Lateral leaflets when present only less than half the size of the terminal ones. Flowers small, brownish red, with short pedicel seen in fascicles of 2-3. Calyx campanulate, glandular, hairy and 4-5 lobed. Corolla with brownish red, broadly linear petals reflexed at apex. Stamens 8-10, alternatively long and short. Ovary oblong, ovoid and stigma bifid. Fruit is a drupe and red when ripe, ovate in shape with 2-3 celled stones. The chromosome number 2n= 26 (Warrier et al, 1994; Tajuddin et al, 1994).

Agrotechnology: Guggal being a plant of arid zone thrives well in arid- subtropical to tropical climate.

The rainfall may average between 100mm and 500mm while air temperature may vary between 40 C in summer and 3 C during winter. Maximum relative humidity prevails during rainy season (83% in the morning and 48% in the evening).Wind velocity remains between 20-25 km/hour during the year is good. Though they prefer hard gypseous soil, they are found over sandy to silt loam soils, poor in organic matter but rich in several other minerals in arid tracks of western India (Tajuddin et al, 1994).

Plants are propagated both by vegetatively and seeds. Plants are best raised from stem cuttings from the semi woody (old) branch. For this purpose one metre long woody stem of 10mm thickness is selected and the cut end is treated with IBA or NAA and planted in a well manured nursery bed during June-July months; the beds should be given light irrigation periodically. The cuttings initiate sprouting in 10-15 days and grow into good green sprout in 10-12 months. These rooted plants are suitable for planting in the fields during the next rainy season. The cuttings give 80-94% sprouting usually. Air layering has also been successfully attempted and protocol for meristem culture is available in literature. Seed germination is very poor (5%) but seedling produce healthier plants which withstand high velocity winds.

The rooted cuttings are planted in a well laid-out fields during rainy season. Pits of size 0.5m cube are dug out at 3-4 m spacing in rows and given FYM and filler soil of the pit is treated with BHC (10%) or aldrin (5%) to protect the new plants from white ants damage. Fertilizer trials have shown little response except due to low level of N fertilization. Removal of side branches and low level of irrigation supports a good growth of these plants. The plantation does not require much weeding and hoeing. But the soil around the bushes be pulverised twice in a year to increase their growth and given urea or ammonium sulphate at 25- 50g per bush at a time and irrigated. Dalal et al (1989) reported that cercospora leaf spot was noticed on all the cultures. Bacterial leaf blight was also noticed to attack the cultures. A leaf eating caterpillar (Euproctis lanata Walker) attack guggal, though not seriously. White fly (Bemisia tabaci) is observed to suck sap of leaves and such leaves become yellowish and eventually drop. These can be effectively controlled by using suitable insecticide.

Stem or branch having maximum diameter of about 5cm at place of incision, irrespective of age is tapped. The necrotic patch on the bark is peeled off with a sharp knife and Bordeaux paste is applied to the exposed (peeled off) surface of the stem or branch. A prick chisel of about 3cm width is used to make bark- deep incisions and while incising the bark, the chisel is held at an acute angle so that scooped suspension present on the body of the chisel flows towards the blade of the chisel and a small quantity of suspension flows inside the incised bark. If tapping is successful, gum exudation ensures after about 15-20 days from the date of incision and continues for nearly 30-45 days. The exuded gum slides down the stem or branch, and eventually drops on the ground and gets soiled. A piece of polythene sheet can be pouched around the place of incision to collect gum. Alternatively, a polythene sheet can be spread on the ground to collect exuded gum. A maximum of about 500g of gum has been obtained from a plant (Dalal, 1995).

Post harvest technology: The best grade of guggul is collected from thick branches of tree. These lumps of guggul are translucent. Second grade guggul is usually mixed with bark, sand and is dull coloured guggul. Third grade guggul is usually collected from the ground which is mixed with sand, stones and other foreign matter. The final grading is done after getting cleansed material. Inferior grades are improved by sprinkling castor oil over the heaps of the guggul which impart it a shining appearance (Tajuddin et al, 1994).

Properties and activity: The gum resin contains guggul sterons Z and E, guggul sterols I-V, two diterpenoids- a terpene hydrocarbon named cembreneA and a diterpene alcohol- mukulol, -camphrone and cembrene, long chain aliphatic tetrols- octadecan-1,2,3,4-tetrol, eicosan-1,2,3,4-tetrol and nonadecan-1,2,3,4-tetrol. Major components from essential oil of gum resin are myrcene and dimyrcene. Plant without leaves, flowers and fruits contains myricyl alcohol, -sitosterol and fifteen aminoacids. Flowers contain quercetin and its glycosides as major flavonoid components, other constituents being ellagic acid and pelargonidin glucoside (Patil et al, 1972; Purushothaman and Chandrasekharan, 1976).

The gum resin is bitter, acrid, astringent, thermogenic, aromatic, expectorant, digestive, anthelmintic, antiinflammatory, anodyne, antiseptic, demulcent, carminative, emmenagogue, haematinic, diuretic, lithontriptic, rejuvenating and general tonic. Figwort Tea

Mostly known as a cleansing herb, figwort tea was used in the past to treat tuberculosis. Some of the benefits of figwort tea are the detoxifying properties and the capacity to treat skin conditions. About Figworth tea Botanically called Scrophularia nodosa, figwort is also known as carpenter’s square, knotted figwort, throatwort or rose noble. Figwort is a perennial plant with thick square fleshy stems and green or purple flowers, commonly found in the cooler woodlands of Europe, North America and Central Asia. It is harvested in the summer when it is in bloom. It was used in the past to treat tuberculosis commonly called scrofula. It is commonly known as a powerful diuretic and a detoxifier. In China it is associated with salt and taken as a yin tonic. Figwort is typically prepared as a tea infusion, a tincture or as a compress. Figwort tea is prepared from the aerial parts and the roots. Figwort tea contains saponins, cardioactive, glycosides, flavonoids, resin, sugar and organic acids. Ho to prepare figwort tea To make figwort tea, use two teaspoons of dried figwort herbs in a cup of boiling water. Take it out of the heat, and let the mix infuse for about 10 - 15 minutes. It is recommended to be taken not more than twice a day. Benefits of figwort tea Figwort tea is used externally to treat skin conditions, such as eczema and psoriasis. It may also help heal wounds, ulcers, burns, and hemorrhoids. In homeopathic medicine, figwort tea is used to treat decreased resistance, tonsillitis, and lymph edema. It is used internally for its mild laxative effect and its mild diuretic and heart strengthening properties. Figwort tea may stimulate the lymphatic system. Some people believe that figwort Tea may have anti tumor properties. Penicillin

The name given by Sir Alexander Fleming, in 1929, to an antibacterial substance produced by the mould Penicillium notatum. The story of penicillin is one of the most dramatic in the history of medicine, and its introduction into medicine initiated a new era in therapeutics comparable only to the introduction of ANAESTHESIA by Morton and Simpson and of ANTISEPTICS by Pasteur and Lister. The two great advantages of penicillin are that it is active against a large range of bacteria and that, even in large doses, it is non-toxic. Penicillin di?uses well into body tissues and ?uids and is excreted in the urine, but it penetrates poorly into the cerebrospinal ?uid.

Penicillin is a beta-lactam antibiotic, one of a group of drugs that also includes CEPHALOSPORINS. Drugs of this group have a four-part beta-lactam ring in their molecular structure and they act by interfering with the cell-wall growth of mutliplying bacteria.

Among the organisms to which it has been, and often still is, active are: streptococcus, pneumococcus, meningococcus, gonococcus, and the organisms responsible for syphilis and for gas gangrene (for more information on these organisms and the diseases they cause, refer to the separate dictionary entries). Most bacteria of the genus staphylococcus are now resistant because they produce an enzyme called PENICILLINASE that destroys the antibiotic. A particular problem has been the evolution of strains resistant to methicillin – a derivative originally designed to conquer the resistance problem. These bacteria, known as METHICILLINRESISTANT STAPHYLOCOCCUS AUREUS (MRSA), are an increasing problem, especially after major surgery. Some are also resistant to other antibiotics such as vancomycin.

An important side-e?ect of penicillins is hypersensitivity which causes rashes and sometimes ANAPHYLAXIS, which can be fatal.

Forms of penicillin These include the following broad groups: benzylpenicillin and phenoxymethyl-penicillin; penicillinase-resistant penicillins; broad-spectrum penicillins; antipseudomonal penicillins; and mecillinams. BENZYLPENICILLIN is given intramuscularly, and is the form that is used when a rapid action is required. PHENOXYMETHYLPENICILLIN (also called penicillin V) is given by mouth and used in treating such disorders as TONSILLITIS. AMPICILLIN, a broad-spectrum antibiotic, is another of the penicillins derived by semi-synthesis from the penicillin nucleus. It, too, is active when taken by mouth, but its special feature is that it is active against gram-negative (see GRAM’S STAIN) micro-organisms such as E. coli and the salmonellae. It has been superceded by amoxicillin to the extent that prescriptions for ampicillin written by GPs in the UK to be dispensed to children have fallen by 95 per cent in the last ten years. CARBENICILLIN, a semi-synthetic penicillin, this must be given by injection, which may be painful. Its main use is in dealing with infections due to Pseudomonas pyocanea. It is the only penicillin active against this micro-organism which can be better dealt with by certain non-penicillin antibiotics. PIPERACILLIN AND TICARCILLIN are carboxypenicillins used to treat infections caused by Pseudomonas aeruginosa and Proteus spp. FLUCLOXACILLIN, also a semi-synthetic penicillin, is active against penicillin-resistant staphylococci and has the practical advantage of being active when taken by mouth. TEMOCILLIN is another penicillinase-resistant penicillin, e?ective against most gram-negative bacteria. AMOXICILLIN is an oral semi-synthetic penicillin with the same range of action as ampicillin but less likely to cause side-effects. MECILLINAM is of value in the treatment of infections with salmonellae (see FOOD POISONING), including typhoid fever, and with E. coli (see ESCHERICHIA). It is given by injection. There is a derivative, pivmecillinam, which can be taken by mouth. TICARCILLIN is a carboxypenicillin used mainly for serious infections caused by Pseudomonas aeruginosa, though it is also active against some gram-negative bacilli. Polyscias Fruticosa

(L.) Harms.

Synonym: Nothopanaxfruticosum (L.) Miq.

Panax fruticosus L.

Family: Araliaceae.

Habitat: Cultivated in gardens all over India.

Action: Leaf—used in sinusitis, headache, migraine, tonsillitis. Stem bark—used for promoting expulsion of placenta after child birth. Root— antibacterial, antifungal, diuretic. Leaf and root—used in dysuria.

The root contains polyacetylenes, falcarinol and heptadeca derivatives. Falcarinol and heptadeca exhibited strong antibacterial activity against Gram-positive bacteria and the der- matophytic bacteria, also showed an- tifungal activity. The antibacterial activity of falcarinol was found to be 15 to 35 times stronger than that of erythromycin, chloramphenicol and oxytetracyclin.

Polyscias scutellaria (Burm. f.) F. R. Fosberg (commonly grown in Indian gardens) exhibits anti-inflammatory activity. The leaves contain several tri- terpenoid saponins, polyscisaponins, oleanolic acid derivatives.... polyscias fruticosa

Adenitis

Inflammation of lymph nodes. Cervical adenitis (swelling and tenderness of the lymph nodes in the neck) occurs in certain bacterial infections, especially tonsillitis, and glandular fever (see infectious mononucleosis). Mesenteric lymphadenitis is inflammation of the lymph nodes inside the abdomen and is usually caused by viral infection. Penicillin Drugs

A group of antibiotic drugs.

Natural penicillins are derived from the mould PENICILLIUM; others are synthetic preparations.

Penicillins are used to treat many infective conditions, including tonsillitis, bronchitis, bacterial endocarditis, syphilis, and pneumonia.

They are also given to prevent rheumatic fever from recurring.

Common adverse effects of penicillins are an allergic reaction causing a rash, and diarrhoea.... penicillin drugs

Phenoxymethylpenicillin

A synthetic penicillin drug, which is an antibiotic prescribed to treat bacterial infections including pharyngitis, tonsillitis, and tooth abscess.

Possible adverse effects include rash and nausea.

Salvia Officinalis

Linn.

Family: Labiatae; Lamiaceae.

Habitat: Native to the Mediterranean region; grown as an ornamental.

English: Sage.

Folk: Salvia Sefakuss.

Action: Plant—astringent, anti- inflammatory, carminative, anti- spasmodic, antiseptic. Leaf and flower—cholagogue, hypogly- caemic, antiasthmatic (used for respiratory allergy), cholagogue, emmenagogue, antisudoriferous, antiseptic. Leaf—diaphoretic, antipyretic. Used for sore throat, laryngitis, tonsillitis, stomatitis.

Key application: Leaf—internally, for dyspeptic symptoms and excessive perspiration; externally for inflammations of the mucous membranes of nose and throat. (German Commission E.) ESCOP indicates its use for inflammations and infections such as stomatitis, gingivitis, pharyngitis, and hyperhidrosis.

The leaves contain a volatile oil; diterpene bitters including carnosolic acid; flavonoids including salvigenin, genkwanin, hispidulin, luteolin and its derivatives; phenolic acids including rosmarinic, caffeic, labiatic; a condensed catechin, salvia tannin.

The roots contain diterpene quino- nesroyleanone and its derivatives. Volatile oil contains alpha-and beta-thu- jone, 1,8-cineole and camphor. Thu- jone is strongly antiseptic and carminative, also has an oestrogenic action that is partly responsible for the herb's hormonal activity in reducing breast milk production. The volatile oil also relieves muscle spasms. Ros- marinic acid, a phenol, allays inflammations.

Cirsiliol, linalool and alpha-terpine- ol, constituents of the volatile oil, exhibit CNS depressant activities.

In a double blind, randomized and placebo controlled trial, extracts of Salvia officinalis showed improvement in patients with mild to moderate Alzheimer disease. (Natural Medicines Comprehensive Database, 2007.)

Sage oil is used in perfumes as a deodorant and for the treatment of thrush and gingivitis. The herb is used in tooth powders, mouth washes, gargles, poultices, hair tonics and hair dressings.... salvia officinalis

Abdominal Pain

(Acute). Sudden unexplained colicky pain with distension in a healthy person justifies immediate attention by a doctor or suitably trained practitioner. Persistent tenderness, loss of appetite, weight and bowel action should be investigated. Laxatives: not taken for undiagnosed pain. Establish accurate diagnosis.

The following are brief indications for action in the absence of a qualified practitioner. Flatulence (gas in the intestine or colon), (Peppermint). Lower left – diverticulitis, colitis, (Fenugreek seeds). Female organs, (Agnus Castus). Kidney disorders, (Buchu). Bladder, (Parsley Piert). Hiatus hernia (Papaya, Goldenseal). Cudweed, Marsh

Preparations: Thrice daily.

Tea. 1 heaped teaspoon to each cup boiling water; infuse 15 minutes, half cup freely, acute cases; thrice daily, chronic. Addition of 2 drops Tincture Myrrh enhances action.

Powder: dose, 2-4g.

Liquid Extract: half-1 teaspoon in water.

Respiratory Tract Infection

Infection of the breathing passages, which extend from the nose to the alveoli. This type of infection is divided into upper and lower respiratory tract infections. Upper respiratory tract infections affect the nose, throat, sinuses, and larynx. They include the common cold, pharyngitis, tonsillitis, sinusitis, laryngitis, and croup. Lower respiratory tract infections, which affect the trachea, bronchi, and lungs, include acute bronchitis, acute bronchiolitis, and pneumonia.... respiratory tract infection

Streptococcal Infections

Infections caused by bacteria of the STREPTOCOCCUS group.

A particular type, haemolytic streptococci, can cause tonsillitis, strep throat, scarlet fever, otitis media, pneumonia, erysipelas, and wound infections.

Another type is often responsible for urinary tract infection, and another can cause bacterial endocarditis if it enters the bloodstream.... streptococcal infections

Tonsil

One of a pair of oval tissue masses at the back of the throat on either side. The tonsils are made up of lymphoid tissue and form part of the lymphatic system. Along with the adenoids, at the base of the tongue, the tonsils protect against upper respiratory tract infections. The tonsils gradually enlarge

from birth until the age of 7, after which time they shrink substantially.

Tonsillectomy

Surgical removal of the tonsils, which is now performed only if a child suffers frequent, recurrent attacks of severe tonsillitis. The operation is also carried out to treat quinsy (an abscess around the tonsil).... tonsillectomy

Trismus

Involuntary contraction of the jaw muscles, which causes the mouth to become tightly closed.

Commonly known as lockjaw, it may occur as a symptom of tetanus, tonsillitis, mumps, or acute ulcerative gingivitis and other dental problems affecting the back teeth.

Treatment is of the underlying cause.... trismus

Children

Massive and long continued medication should be avoided, parents acquiring some ability to distinguish between the purely miserable and the critically ill. It is easy to become alarmed at the sight of a child in the throes of a convulsion or feverishness when there may be a tendency to over-prescribe. German Chamomile tea is a splendid children’s remedy. Liquid Extract and Tincture doses for children are 1 drop and 2 drops, respectively, for each year of age.

Anti-depressants should not be given for bed-wetting, drugs for sleep problems or strong laxatives for the chronically constipated. Mild herbal alternatives exist. Fresh carrot juice daily helps a child to avoid some complaints. Some herbs are not advised for children under 12, except under the care of a qualified practitioner.

Parental smoking habits are known to be responsible for crying and digestive symptoms in infants. Sleeplessness. German Chamomile or Balm tea: children 2-10 years quarter to half a cup; over 10 years: 1 cup. Babies: 3-6 teaspoons in feeding bottle – sweeten with honey if necessary.

Night seizures, with screaming: Passion Flower tea. 1 heaped teaspoon to cup boiling water; infuse 5-15 minutes. Strain. A few teaspoons at bedtime. When a brain storm starts place pinch of salt on the tongue. Calcium deficiency. Nettle tea. Carrot juice. Cod Liver oil with fresh orange juice.

Colic. Any tea: Dill, Catnep, Spearmint or Fennel. Few teaspoons frequently. Abdominal massage: 3 drops Chamomile oil in teaspoon olive oil.

Constipation. Prune or carrot juice. Dandelion coffee.

Cough. Oil of Thyme – few drops in water.

Crusta Lacta (milk rash). Weak teas: Plantain, Heartsease, Red Clover. Anoint with St John’s Wort oil. Buttermilk, Wheatgerm.

Diarrhoea. Teas: Yarrow, Tormentil. Breast feeding during the first 4-6 months of life reduces the risk of children’s diarrhoea.

Digestion, weak. Teas: Fennel, Caraway, Dill. 1 teaspoon crushed seeds to cup boiling water. Infuse 15 minutes in a covered vessel. Teaspoon doses for under 2s; half-1 cup thereafter. Also for flatulence. Feverishness. Alarm at a baby’s fever and fractiousness may attract complete medical treatment including nose drops, cough linctus, antipyretics and antibiotics, together with something to let the parents get some sleep. Avoid where possible. Mild fevers: teas – Yarrow, Marigold, Thyme, Elderflowers and Peppermint, Catmint, Carragheen Moss. Sweeten with honey. Topical: Flannels wrung out in these hot teas. Zinc can cut short the common cold. Echinacea tablets/capsules offer antiviral protection.

It is common for a child to convulse with fever. A feverish child, kept cool, is less likely to have convulsions. Remove most of child’s clothes so he can lose heat through the skin. Fruit juices (Vitamin C) in abundance. Do not feed solid foods. Wash in lukewarm (not cold) water.

Eyes. Deep hollows under the eyes reveal exhaustion, for which blood and nerve tonics and iron supplements are indicated.

Growth problems. Under-developed children respond well to herbal aids: Gentian, Ginseng, Horsetail, Marigold, Oats, German Chamomile, Wood Betony, Kelp, Alfalfa. Supplementation with brewer’s yeast, Calcium, Pollen and Zinc yield convincing results.

Hyperactivity. Nerve restoratives for highly-strung children: Teas: Lime flowers, Chamomile, Lemon Balm, B-vitamins. Porridge. Tablets: Passion flower, Valerian, Skullcap. Vitamins B6 and C. Powders: formula. Passion flower 2; Valerian 1; Liquorice 1. Dose: 250mg (one 00 capsule or one-sixth teaspoon) thrice daily.

Irritability and impaired school performance may be due to Tartrazine and other additives, sugar, and anticonvulsant drugs. See previous paragraph.

Infection. Infection of the upper respiratory tract may manifest as inflammation of the middle ear, nasal discharge or tonsillitis. Echinacea tablets, powder or liquid extract indicated. For specific infection such as measles, see under MEASLES, or other appropriate entry.

Elder

Black Elder. Sambucus nigra L. Flowers, bark, berries. German: Holunder. French: Sureau. Spanish: Sauco. Italian: Sambreo.

Constituents: flavonoids, oil, tannins.

Berries contain Vitamin C and iron. Elderblossom works well with Peppermint or Yarrow, as a tea. Action: anti-inflammatory, laxative (especially berries and bark), anticatarrhal, relaxing diaphoretic, hydragogue (inner bark), cathartic (inner bark). Elderblossom is an emollient skin care product. Emetic (inner bark). Diuretic (urinary antiseptic). An ancient household remedy for promoting flow of urine (cold infusion). Expectorant (hot infusion).

Uses: the common cold, influenza, winter’s chills, early stages of fevers with dry skin and raised body temperature. Nasal catarrh, sinusitis. Tonsillitis, inflammation of mouth, throat and trachea (mouth wash and gargle). Night sweats (cold infusion). Chilblains (local).

“The inner bark of Elder has been used with success in epilepsy by taking suckers or branches 1-2 years old. The grey outer bark is scraped off and 2oz of it steeped in 5oz boiling water for 48 hours. Strain. Give a wineglassful every 15 minutes when a fit is threatening. Have the patient fast. Resume every 6 to 8 days.” (Dr F. Brown (1875))

Croup (combined with Coltsfoot – equal parts). Eyestrain, conjunctivitis, twitching: cotton wool pads soaked in cold Elder tea applied to the closed lids, patient lying down.

Preparations: Tea (flowers) 2 teaspoons (2-4g) in each cup boiling water; infuse 5 minutes. Half-1 cup two-hourly for acute conditions. Cold tea is laxative and sedative. Hot tea excites and stimulates. Cold tea soothes and heals chapped hands and useful for sunbathing.

Distilled Elderflower water: for inflamed eyes.

Liquid Extract. 1 teaspoon in water, thrice daily.

Home tincture (traditional). Chippings of inner green bark macerated in white wine for 8 days, strain; for dropsy and constipation.

Ointment. 3 parts fresh Elder leaves. Heat with 6 parts Vaseline until leaves are crisp; strain and store. (David Hoffmann)

Elderberry wine: traditional.

Lymphadenitis

n. inflammation of lymph nodes, which become swollen, painful, and tender. Some cases may be chronic (e.g. tuberculous lymphadenitis) but most are acute and localized adjacent to an area of infection. The most commonly affected lymph nodes are those in the neck, in association with tonsillitis. The lymph nodes help to contain and combat the infection. Occasionally generalized lymphadenitis occurs as a result of virus infections. The treatment is that of the cause.... lymphadenitis

Echinacea

Cone flower, Black Sampson. Echinacea pallida, Nutt. Echinacea angustifolia (DC) Heller. Brauneria pallida, Nutt. Echinacea purpurea. Part used: rhizome and whole of the plant. Constituents: Echinacosides (in Echinacea angustifolia), alkaloids, polysaccharides, flavonoids, essential oil.

Action. Antimicrobial, antiseptic, anti-inflammatory, tonic, detoxicant, parasiticide, antibiotic (non- toxic), vasodilator, lymphatic. Does not act directly upon a virus but exerts an antiviral effect by stimulating an immune response. Raises white blood cell count and increases the body’s inherent powers of resistance. Has power to stimulate ‘killer’ cells that resist foreign bacteria. T-cell activator. Vulnerary. Uses. Boils, acne, abscesses, sore throat: streptococcal and staphylococcal infections generally. Ulcers of tongue, mouth, gums, tonsils, throat (mouth wash and gargle). Duodenal and gastric ulcer. Systemic candida. Putrefaction and fermentation in the alimentary tract. Skin disorders: eczema. Infection of the fallopian tubes. Ill-effects of vaccination. A cleansing wash and lotion for STDs and varicose ulcers. Vaginal candidiasis.

Historical. “Many years ago American Indians observed that by tantalising the rattlesnake it would in its wrath bite itself. The creature was seen to become immediately restless and sought to retreat. On following the snake it was observed that it went straight to a certain shrub and there became a veritable ‘sucker’. When it finished sucking the plant it would seek a hole in which to hide, but not to die. It would recover. This led to the discovery of the plant, Echinacea. It was from the medicine-men of the Mohawk and Cherokee Indians we obtained our first knowledge of this remarkable herbal remedy.” (J.H. Henley MD, Enid, Oklahoma, USA)

Lovage

Constituents: coumarins, butyric acid, volatile oil.

Action: antibiotic (mild), diaphoretic, expectorant, anti-catarrhal, emmenagogue, carminative, diuretic (mild), sedative, antispasmodic.

Uses: flatulent dyspepsia, anorexia, rheumatism, gout, absent or painful menses, mild feverishness in children, renal dropsy (mild), cystitis.

Locally: as a gargle for tonsillitis; mouth wash for mouth ulcers BHP (1983).

Combinations. With Agrimony for indigestion. With Buchu for renal dropsy. With Raspberry leaves for menstrual disorders. Usually combined with other diuretics.

Preparations: Average dose: half-2g. Thrice daily.

Decoction. Half-1 teaspoon to each cup water gently simmered 15 minutes. Dose: one-third-1 cup.

Liquid Extract: 5-30 drops in water.

Oil used in aromatherapy and perfumes.

LOZENGE. A compressed tablet for sucking in the mouth for inflammatory conditions of throat, mouth and chest. Usually has a base of sugar, a demulcent (Comfrey, Marshmallow root, Slippery Elm, Tolu, or Balm of Gilead), together with a binder, such as gum acacia or gum tragacanth. Recipe for simple lozenge: half an ounce (15g) powdered herb; half an ounce sugar; powdered gum 1 teaspoon (3g). Pure spring water – a sufficiency. Pharyngitis

n. inflammation of the part of the throat behind the soft palate (pharynx). It produces *sore throat and may be associated with *tonsillitis.... pharyngitis

Scarlet Fever

a highly infectious disease caused by a strain of *Streptococcus bacteria that produces toxins. Symptoms begin 2–4 days after exposure and include fever, tonsillitis, and a characteristic widespread scarlet rash. The tongue is also affected; initially covered by a thick white material, it then becomes bright red (the ‘strawberry tongue’). Treatment with antibiotics shortens the course of the disease and reduces the risk of secondary complications, which include kidney and ear inflammation. Medical name: scarlatina. First Aid And Medicine Chest

Various aspects of first aid are described under the following: ABRASIONS, BLEEDING, CUTS, SHOCK, EYES, FAINTING, FRACTURES, INJURIES, POISONING, WOUNDS, WITCH HAZEL.

Avoid overstocking; some herbs lose their potency on the shelf in time, especially if exposed. Do not keep on a high shelf out of the way. Experts suggest a large box with a lid to protect its contents, kept in a cool dry place away from foods and other household items. Store mixtures containing Camphor separately elsewhere. Camphor is well-known as a strong antidote to medicinal substances. Keep all home-made ointments in a refrigerator. However harmless, keep all remedies out of reach of children. Be sure that all tablet containers have child-resistant tops.

Keep a separate box, with duplicates, permanently in the car. Check periodically. Replace all tablets when crumbled, medicines with changed colour or consistency. Always carry a large plastic bottle of water in the car for cleansing dirty wounds and to form a vehicle to Witch Hazel and other remedies. Label all containers clearly.

Health care items: Adhesive bandages of all sizes, sterile gauze, absorbant cotton wool, adhesive tape, elastic bandage, stitch scissors, forceps (boiled before use), clinical thermometer, assorted safety pins, eye-bath for use as a douche for eye troubles, medicine glass for correct dosage.

Herbal and other items: Comfrey or Chickweed ointment (or cream) for sprains and bruises. Marshmallow and Slippery Elm (drawing) ointment for boils, abscesses, etc. Calendula (Marigold) ointment or lotion for bleeding wounds where the skin is broken. An alternative is Calendula tincture (30 drops) to cupful of boiled water allowed to cool; use externally, as a mouth rinse after dental extractions, and sipped for shock. Arnica tincture: for bathing bruises and swellings where the skin is unbroken (30 drops in a cup of boiled water allowed to cool). Honey for burns and scalds. Lobelia tablets for irritating cough and respiratory distress. Powdered Ginger for adding to hot water for indigestion, vomiting, etc. Tincture Myrrh, 5-10 drops in a glass of water for sore throats, tonsillitis, mouth ulcers and externally, for cleansing infected or dirty wounds. Tincture Capsicum (3-10 drops) in a cup of tea for shock, or in eggcup Olive oil for use as a liniment for pains of rheumatism. Cider vinegar (or bicarbonate of Soda) for insect bites. Oil Citronella, insect repellent. Vitamin E capsules for burns; pierce capsule and wipe contents over burnt area. Friar’s balsam to inhale for congestion of nose and throat. Oil of Cloves for toothache. Olbas oil for general purposes. Castor oil to assist removal of foreign bodies from the eye. Slippery Elm powder as a gruel for looseness of bowels. Potter’s Composition Essence for weakness or collapse. Antispasmodic drops for pain.

Distilled extract of Witch Hazel deserves special mention for bleeding wounds, sunburn, animal bites, stings, or swabbed over the forehead to freshen and revive during an exhausting journey. See: WITCH HAZEL.

Stings of nettles or other plants are usually rendered painless by a dock leaf. Oils of Tea Tree, Jojoba and Evening Primrose are also excellent for first aid to allay infection. For punctured wounds, as a shoemaker piercing his thumb with an awl or injury from brass tacks, or for shooting pains radiating from the seat of injury, tincture or oil of St John’s Wort (Hypericum) is the remedy. ... first aid and medicine chest

Oak

English oak. Quercus robur L. Dried inner bark. German: Stieleiche. French: Bouvre. Italian: Rovere.

Constituents: tannins, quercin, pectin, resin.

Action: astringent, styptic, antiseptic, anti-inflammatory, haemostatic.

Uses: Alcoholism, diarrhoea, dysentery, colitis. To cleanse external ulcers and suppurating wounds. Mouth ulcers, spongy gums, sore throat, tonsillitis, (gargle and mouth-wash). Non-infectious vaginal discharge, leucorrhoea, (douche). Chilblains (decoction as a lotion). Piles (decoction as an enema). Preparations. Average dose, half-2 grams, or equivalent. Thrice daily.

Decoction. Half an ounce to 1 pint water gently simmered 20 minutes. Dose, one-third to half a cup (internally).

Liquid extract BHP (1983). 1:1 in 25 per cent alcohol; dose, 1-2ml.

Tincture of acorns. Dehusk and pulverise acorns. 1 part to 5 parts 45 per cent alcohol (vodka etc). Macerate 8 days, shake daily. Filter. Dose, 15-30 drops. External: 1 part to 20 parts boiled water. Powder, inner bark or acorns, for dusting foul-smelling ulcers and septic wounds.

Oak Bath. 6-8oz bark to 10 litres (7 pints) water simmered 20 minutes. Add to bath water. Oak Compress. Soak a piece of lint in decoction and fix firmly for acute eye troubles. ... oak

Otitis Media

Inflammation of the middle ear. Usually spreads from the nose or throat via the Eustachian tube. Tonsillitis, sinusitis or ‘adenoids’ predispose. A frequent complication of measles, influenza or other children’s infections. Sometimes due to allergy.

Symptoms. Effusion of fluid into the middle ear with increasing deafness, discharge, tinnitus. Infant shakes head. Perforation in chronic cases. Inspection with the aid of an auriscope reveals bulging of the ear-drum. Feverishness.

Treatment. Antibiotics (herbal or others) do not remove pain therefore a relaxing nervine should be included in a prescription – German Chamomile, Vervain, etc.

Before the doctor comes. Any of the following teas: Boneset, Feverfew, Holy Thistle, Thyme. One heaped teaspoon to each cup boiling water; infuse 15 minutes; one cup thrice daily.

Formula. Practitioner. Echinacea 2; Thyme 1; Hops half; Liquorice quarter. Dose – Powders: 500mg (two 00 capsules or one-third teaspoon). Liquid Extracts: 1 teaspoon. Tinctures: 1-2 teaspoons. Acute: every 2 hours. Chronic: thrice daily.

Topical. Dry-mop purulent discharge before applying external agents. Inject warm 2-3 drops any one oil: Mullein, St John’s Wort, Garlic, Lavender or Evening Primrose.

Once every 8-10 days syringe with equal parts warm water and Cider Vinegar. Repeat cycle until condition is relieved.

Diet. Salt-free. Low-starch. Milk-free. Abundance of fruits and raw green salad vegetables. Freshly squeezed fruit juices. Bottled water. No caffeine drinks: coffee, tea or cola.

Supplements. Vitamins A, B-complex, B2, B12, C, E, K, Iron, Zinc. Evening Primrose capsules.

Notes. Where pressure builds up against the drum, incision by a general medical practitioner may be necessary to facilitate discharge of pus. Grossly enlarged tonsils and adenoids may have to be surgically removed in chronic cases where treatment over a reasonable period proves ineffective. A bathing cap is sometimes more acceptable than earplugs.

Breast-feeding. Significantly protects babies from episodes of otitis media. Commenting on a study published in the Obstetrical and Gynaecological Survey, Dr Mark Reynolds, author of a breast-feeding policy by the Mid-Kent Care Trust said: “Breast milk is known to reduce respiratory infection – a precursor of otitis media.”

Earache

Pain in the ear. Earache is a common symptom, especially in childhood. The most frequent cause is acute otitis media, which results in severe, stabbing pain. Another common cause of earache is otitis externa. The pain may be accompanied by irritation and a discharge of pus. Intermittent earache may accompany dental problems, tonsillitis, throat cancer (see pharynx, cancer of), or pain in the jaw or neck muscles.

To determine the cause of earache, the ear is inspected (see ear, examination of). Analgesic drugs may relieve the pain, and antibiotic drugs may be given for infection. Pus in the outer ear may be removed by suction. Pus in the middle ear may be drained by myringotomy. ear, cauliflower See cauliflower ear. ear, discharge from An emission of fluid from the ear, also called otorrhoea. It may be due to outer-ear infection (see otitis externa). It may also follow perforation of the eardrum (see eardrum, perforated), usually due to middle-ear infection (see otitis media). Rarely, after a skull fracture, cerebrospinal fluid or blood may be discharged.

A swab may be taken of the discharge and analysed to identify any infection. Hearing tests may be performed. X-rays of the skull are taken if there has been a head injury or serious middle-ear infection is suspected. Treatment usually includes antibiotic drugs.... earache

Fever

Elevation above normal of body temperature. Normal body temperature is 37oC in the mouth and 0.6oC lower in the axilla (armpit). A fever may be accompanied by symptoms such as shivering, headache, sweating, thirst, faster-thannormal breathing, and a flushed face. Confusion or delirium sometimes occur, especially in the elderly; a high fever may cause seizures in a child under 5 years (see convulsion, febrile) or coma.

Most fevers are caused by a bacterial infection such as tonsillitis or a viral infection such as influenza. In these cases, proteins called pyrogens are released when the white blood cells fight the microorganisms that are responsible for the infection. Pyrogens act on the temperature controlling centre in the brain, causing it to raise the body temperature in an attempt to destroy the invading microorganisms. Fever may also occur in conditions, such as dehydration, thyrotoxicosis, lymphoma, and myocardial infarction, where infection is not present.

Drugs such as aspirin or other nonsteroidal anti-inflammatory drugs, or paracetamol may be given to reduce fevers that are due to infections.

Otherwise, treatment is directed at the underlying cause (for example, giving antibiotic drugs for a bacterial infection).... fever

Bergamot

Citrus bergamia

FAMILY: Rutaceae

SYNONYM: Citrus aurantium subsp. bergamia.

GENERAL DESCRIPTION: A small tree, about 4.5 metres high with smooth oval leaves, bearing small round fruit which ripen from green to yellow, much like a miniature orange in appearance.

DISTRIBUTION: Native to tropical Asia. Extensively cultivated in Calabria in southern Italy and also grown commercially on the Ivory Coast.

OTHER SPECIES: Not to be confused with the herb bergamot or bee balm (Monarda didyma).

HERBAL/FOLK TRADITION: Named after the Italian city of Bergamo in Lombardy, where the oil was first sold. The oil has been used in Italian folk medicine for many years, primarily for fever (including malaria) and worms; it does not feature in the folk tradition of any other countries. However, due to recent research in Italy, bergamot oil is now known to have a wide spectrum of applications, being particularly useful for mouth, skin, respiratory and urinary tract infections.

ACTIONS: Analgesic, anthelmintic, antidepressant, antiseptic (pulmonary, genito-urinary), antispasmodic, antitoxic, carminative, digestive, diuretic, deodorant, febrifuge, laxative, parasiticide, rubefacient, stimulant, stomachic, tonic, vermifuge, vulnerary.

EXTRACTION: Essential oil by cold expression of the peel of the nearly ripe fruit. (A rectified or terpeneless oil is produced by vacuum distillation or solvent extraction.)

CHARACTERISTICS: A light greenish-yellow liquid with a fresh sweet-fruity, slightly spicy-balsamic undertone. On ageing it turns a brownish-olive colour. It blends well with lavender, neroli, jasmine, cypress, geranium, lemon, chamomile, juniper, coriander and violet.

PRINCIPAL CONSTTTUENTS Known to have about 300 compounds present in the expressed oil: mainly linalyl acetate (30–60 per cent), linalol (11–22 per cent) and other alcohols, sesquiterpenes, terpenes, alkanes and furocoumarins (including bergapten, 0.30–0.39 per cent).

SAFETY DATA: Certain furocoumarins, notably bergapten, have been found to be phototoxic on human skin; that is, they cause sensitization and skin pigmentation when exposed to direct sunlight (in concentration and in dilution even after some time!). Extreme care must be taken when using the oil in dermal applications – otherwise a rectified or ‘bergapten-free’ oil should be substituted. Available information indicates it to be otherwise non-toxic and relatively non-irritant.

AROMATHERAPY/HOME: USE

Skin Care: Acne, boils, cold sores, eczema, insect repellent and insect bites, oily complexion, psoriasis, scabies, spots, varicose ulcers, wounds.

Respiratory System: Halitosis, mouth infections, sore throat, tonsillitis.

OTHER USES: Extensively used as a fragrance and, to a degree, a fixative in cosmetics, toiletries, suntan lotions and perfumes – it is a classic ingredient of eau-de-cologne. Widely used in most major food categories and beverages, notably Earl Grey tea.... bergamot

Jaborandi

Pilocarpus jaborandi

FAMILY: Rutaceae

SYNONYMS: Pernambuco jaborandi, P. pennatifolius, iaborandi, jamborandi, arrudo do mato, arruda brava, jamguaraddi, juarandi.

GENERAL DESCRIPTION: A woody shrub up to 2 metres high with a smooth, greyish bark, large brownish-green leathery leaves containing big oil glands and reddish-purple flowers.

DISTRIBUTION: Native to Brazil; other species are found in Paraguay, Cuba, the West Indies and Central America.

OTHER SPECIES: There are many members of the Rutaceae and Piperaceae family known simply as jaborandi, such as Piper jaborandi. Others include maranham jaborandi (P. microphyllus), ceara jaborandi (P. trachylophus) and aracti jaborandi (P. spicatus).There is consequently some confusion about the exact botanical source of the oil.

HERBAL/FOLK TRADITION: Jaborandi induces salivation and most gland secretions; it was also used at one time to promote hair growth. ‘Useful in psoriasis, prurigo, deafness ... chronic catarrh, tonsillitis and particularly dropsy.’ .

ACTIONS: Antiseptic, diaphoretic, emmenagogue, galactagogue, stimulant (nerve).

EXTRACTION: Essential oil by steam distillation from the dried leaflets.

CHARACTERISTICS: An orange or yellow liquid with a sweet-herbaceous fruity odour.

PRINCIPAL CONSTITUENTS: Pilocarpine is the main active constituent; also isopilocarpine, pilocarpidine, methyl nonyl ketone, dipentene and other hydrocarbons.

SAFETY DATA: Oral toxin, skin irritant, abortifacient.

AROMATHERAPY/HOME: USE None.

OTHER USES: Various hypodermic solutions are prepared from pilocarpine: the crude oil is rarely used. Little used in perfumery or flavour work due to toxicity.... jaborandi

Fragonia

Agonis fragrans

FAMILY: Myrtaceae

SYNONYMS: Taxandria fragrans, coarse tea-tree.

GENERAL DESCRIPTION: Agonis is a genus comprising four species, all of which are native to Western Australia. The species generally have fibrous, brown bark, dull green leaves and inflorescences of small, white flowers. They are best known and most readily identified by the powerful peppermint or eucalyptus-like odour emitted when the leaves are crushed or torn. A. fragrans is a small shrub which grows up to 2.5 metres high, with narrow leaves and clusters of small white flowers, characteristic of the genus.

DISTRIBUTION: As a wild native species, fragonia (A. fragrans) has limited distribution in Western Australia, growing near the coast in the south-west region and being reliant on its winter rains and drier summers. For commercial purposes, A. fragrans is grown in large plantations in south-western Australia.

OTHER SPECIES: The Myrtaceae is a large family of plants with over 3,000 species. It is one of the most important families from an aromatherapy perspective, as it includes not only members of the Agonis genus (which includes trees such as A. flexuosa, the Western Australian peppermint) but also hundreds of aromatic plants from the Eucalyptus, Leptospermum, Melaleuca, Myrtus and Pimenta genera. There are several varying chemotypes of A. fragrans, but fragonia essential oil has a unique balance of primary constituents, which imparts its particular therapeutic qualities.

HERBAL/FOLK TRADITION: The name Agonis derives from the Greek agon, meaning ‘gathering’ or ‘collection’, in reference to the tightly clustered flowers. Traditional knowledge on A. fragrans has never been recorded, and there is no known use of the plant by early settlers. The species only came to the forefront at around the turn of the century when a husband-and-wife team heard about the local plant and began to explore its potential. Having selected superior genetic varieties, they established a small plantation of these shrubs on their property in south-west Western Australia in 2001. The essential oil distilled from this specific plant and chemotype is thus relatively new to the aromatherapy industry. Indeed it has only recently been given its common name ‘fragonia’ by its discoverer Chris Robinson, and has since been trademarked as FragoniaTM. In a series of tests, the University of Western Australia demonstrated that fragonia oil has anti-inflammatory properties and significant anti-microbial activity, similar to tea tree oil.

ACTIONS: Analgesic (mild), antibacterial, anti-inflammatory, antifungal, antimicrobial, anti-infectious, antiseptic, expectorant, immuno-tonic, nervine, regulating.

EXTRACTION: An essential oil by steam distillation from the stems, twigs and leaves.

CHARACTERISTICS: A pale, watery liquid with a pleasant slightly citrus, fresh-clean and faintly medicinal top note, mixed with a slight spicy, earthy and balsamic undertone: more pleasing than tea tree. It blends well with niaouli, eucalyptus, myrtle, lemon myrtle, rosemary and tea tree.

PRINCIPAL CONSTITUENTS: Primary constituents are 1,8-cineole, alpha-pinene and linalool. From a chemical perspective, it is an extremely well-balanced oil, with the oxides (1,8-cineole), monoterpenes (alpha-pinene) and monoterpenols (linalool, geraniol, terpineol and others) in a near perfect 1:1:1 ratio.

SAFETY DATA: Fragonia essential oil is non-toxic, non-irritant and non-sensitizing: an extremely mild oil when applied to the skin and safe for children.

Laurel

Laurus nobilis

FAMILY: Lauraceae

SYNONYMS: Sweet bay, bay laurel, Grecian laurel, true bay, Mediterranean bay, Roman laurel, noble laurel, laurel leaf (oil).

GENERAL DESCRIPTION: An evergreen tree up to 20 metres high with dark green, glossy leaves and black berries; often cultivated as an ornamental shrub.

DISTRIBUTION: Native to the Mediterranean region; extensively cultivated especially for its berries, in France, Spain, Italy, Morocco, Yugoslavia, China, Israel, Turkey and Russia. The oil is mainly produced in Yugoslavia.

OTHER SPECIES: There are several related species, all of which are commonly called Bay: Californian bay (Umbellularia california), West Indian bay (Pimenta racemosa) and the cherry laurel (Prunus laurocerasus), which is poisonous.

HERBAL/FOLK TRADITION: A popular culinary herb throughout Europe. The leaves were used by the ancient Greeks and Romans to crown their victors. Both leaf and berry were formerly used for a variety of afflictions including hysteria, colic, indigestion, loss of appetite, to promote menstruation and for fever. It is little used internally these days, due to its narcotic properties. A ‘fixed’ oil of bay, expressed from the berries, is still used for sprains, bruises, earache, etc.

ACTIONS: Antirheumatic, antiseptic, bactericidal, diaphoretic, digestive, diuretic, emmenagogue, fungicidal, hypotensive, sedative, stomachic.

EXTRACTION: Essential oil by steam distillation from the dried leaf and branchlets. (An oil from the berries is produced in small quantities.)

CHARACTERISTICS: A greenish-yellow liquid with a powerful, spicy-medicinal odour. It blends well with pine, cypress, juniper, clary sage, rosemary, olibanum, labdanum, lavender, citrus and spice oils.

PRINCIPAL CONSTITUENTS: Cineol (30–50 per cent), pinene, linalol, terpineol acetate, and traces of methyl eugenol.

SAFETY DATA: Relatively non-toxic and nonirritant; can cause dermatitis in some individuals. Use in moderation due to possible narcotic properties attributed to methyl eugenol. Should not be used during pregnancy.

Geranium

Pelargonium graveolens

FAMILY: Geraniaceae

SYNONYMS: Rose geranium, pelargonium.

GENERAL DESCRIPTION: A perennial hairy shrub up to 1 metre high with pointed leaves, serrated at the edges and small pink flowers. The whole plant is aromatic.

DISTRIBUTION: Native to South Africa; widely cultivated in Russia, Egypt, Congo, Japan, Central America and Europe (Spain, Italy and France). With regard to essential oil production, there are three main regions: Reunion (Bourbon), Egypt and Russia (also China).

OTHER SPECIES: There are over 700 varieties of cultivated geranium and pelargonium, many of which are grown for ornamental purposes. There are several oil-producing species such as P. odorantissimum and P. radens, but P. graveolens is the main one commercially cultivated for its oil. Geranium Bourbon (P. Roseum) has a superior fragrance to other species, having a rich rosy-fruity scent due to the higher levels of linalook and iso-methone content. See also Botanical Classification section.

HERBAL/FOLK TRADITION: The British plant herb robert (Geranium robertianum) and the American cranesbill (G. maculatum) are the most widely used types in herbal medicine today, having been used since antiquity. They have many properties in common with the rose geranium, being used for conditions such as dysentery, haemorrhoids, inflammations, metrorrhagia and menorrhagia (excessive blood loss during menstruation). The root and herb of cranesbill is specifically indicated in the British Herbal Pharmacopoeia for diarrhoea and peptic ulcer.

CHARACTERISTICS: The Bourbon oil is a greenish-olive liquid with a green, rosy-sweet, minty scent. The Bourbon oil is generally preferred in perfumery work; it blends well with lavender, patchouli, clove, rose, sandalwood, jasmine, juniper, neroli, bergamot and other citrus oils.

PRINCIPAL CONSTITUENTS: Citronellol, geraniol, linalol, isomenthone, menthane, phellandrene, sabinene, limonene, among others. Constituents vary according to type and source.

SAFETY DATA: Non-toxic, non-irritant, generally non-sensitizing; possible contact dermatitis in hypersensitive individuals, especially with the Bourbon type.

Hyssop

Hyssopus officinalis

FAMILY: Lamiaceae (Labiatae)

SYNONYM: Azob.

GENERAL DESCRIPTION: An attractive perennial, almost evergreen subshrub up to 60 cms high with a woody stem, small, lance-shaped leaves and purplish-blue flowers.

DISTRIBUTION: Native to the Mediterranean region and temperate Asia; now grows wild throughout America, Russia and Europe. It is mainly cultivated in Hungary and France, and to a lesser degree in Albania and Yugoslavia.

OTHER SPECIES: There are four main subspecies of hyssop, but H. officinalis is the main oil producing variety. The species H. officinalis var. decumbens is less toxic than many other types, and well suited to aromatherapy use. To be distinguished from hedge hyssop (Gratiola officinalis) which is still used in herbal medicine but belongs to an entirely different family.

HERBAL/FOLK TRADITION: Although hyssop is mentioned in the Bible, it probably does not refer to this herb but to a form of wild marjoram or oregano, possibly Oreganum syriacum. Nevertheless H. officinalis has an ancient medical reputation and was used for purifying sacred places, and employed as a stewing herb. ‘The healing virtues of the plant are due to a particular volatile oil.’.

It is used principally for respiratory and digestive complaints, and externally for rheumatism, bruises, sores, earache and toothache. It is also used to regulate the blood pressure, as a general nerve tonic, and for states of anxiety or hysteria. It is current in the British Herbal Pharmacopoeia as a specific for bronchitis and the common cold.

ACTIONS: Astringent, antiseptic, antispasmodic, antiviral, bactericidal, carminative, cephalic, cicatrisant, digestive, diuretic, emmenagogue, expectorant, febrifuge, hypertensive, nervine, sedative, sudorific, tonic (heart and circulation), vermifuge, vulnerary.

CHARACTERISTICS: A colourless to pale yellowy-green liquid with a sweet, camphoraceous top note and warm spicy herbaceous undertone. It blends well with lavender, rosemary, myrtle, bay leaf, sage, clary sage, geranium and citrus oils.

PRINCIPAL CONSTITUENTS: Pinocamphone, isopinocamphone, estragole, borneol, geraniol, limonene, thujone, myrcene, caryophyllene, among others.

SAFETY DATA: Non-irritant, non-sensitizing; the oil is moderately toxic due to the pinocamphone content. It should be used only in moderation and avoided in pregnancy and by epileptics.

Ravintsara

Cinnamomum camphora

FAMILY: Lauraceae

SYNONYMS: Cinnamomum camphora ct. 1,8-cineole.

GENERAL DESCRIPTION: Cinnamomum camphora is a tall evergreen tropical tree characteristic of the Laurel family that grows up to 20–30 metres tall. The foliage is bright green and the leaves have a glossy, waxy appearance bearing masses of small white flowers in spring. It produces clusters of shiny black berries and has a pale bark that is very rough and fissured vertically.

DISTRIBUTION: C. camphora is native to China, Japan, and Taiwan but has been introduced to many countries throughout the world including India, Australia and the USA, where in places such as Florida it is becoming increasingly invasive and a danger to the native flora. This species is not indigenous to Madagascar and was introduced onto the island during the middle of the nineteenth century. As the plant became accustomed to the Malagasy climate it became a distinct subspecies, which in time adapted so well to the new environment, naturalizing in the central-eastern regions, that it became a very sustainable species for producing an essential oil, now called ravintsara oil. However, since it is clear there are many different sub-varieties of C. camphora in various parts of the world, it is possible that there also existed another native aromatic tree already growing in Madagascar originally known as ravintsara by the local people. According to Georges M. Halpern in The Healing Trail: Essential Oils of Madagascar, an indigenous tree of this name had been part of the Malagasy culture for many centuries and its leaves were used as a traditional folk medicine.

OTHER SPECIES: The genus Cinnamomum has approximately 250 different species, many of which yield an essential oil on distillation. Amongst the most important are C. zeylanicum (cinnamon bark and leaf oils) and C. cassia (cassia oil). The single species C. camphora itself produces a wide range of essential oils including: camphor, ho wood and leaf oil, sassafras and ravintsara. All these essential oils, although derived from the same tropical tree or botanical species, differ in their chemical profile enormously depending on the country of origin as well as the part of the plant used to extract the oil: e.g. leaf or bark.

There are at least 6 different chemotypes of C. camphora essential oil due to this factor, including the camphor, linalool, 1,8-cineole, nerolidol, safrole and borneol types. For example, C. camphora, grown in Taiwan and Japan, is known as Ho oil, and has linalool as its major constituent, whereas in India and Sri Lanka the camphor type of C. camphora remains the most important. The variety of C. camphora cultivated in Madagascar contains high levels of 1,8-cineole and no camphor. In order to avoid any confusion, it has been suggested that Ravintsara should be labelled: ‘Cinnamomum camphora ct. 1,8-cineole.’

HERBAL/FOLK TRADITION: The term Cinnamomum is derived from the Greek root kinnamon or kinnamomon meaning ‘sweet wood’. The word ravint means leaf and sara means ‘good for you’ according to the Malagasy tradition, as the local people have long appreciated the therapeutic value of its leaves which were used in folk medicine to treat digestive complaints, headaches, colds and chest infections. It has also been used for culinary applications and employed as a herbal tea. The Malagasy people themselves believe that the original Ravintsara tree is an indigenous species: a ‘cousin’ of the well-known C. camphora species, which has since been introduced.

ACTIONS: Antibacterial, anticatarrhal, antifungal, anti-infectious, antimicrobial, antirheumatic, antiviral, decongestant, cicatrisant (wound healer), expectorant, immune and neurotonic.

EXTRACTION: Steam distilled from the leaves of the plant.

CHARACTERISTICS: Fresh, woody, slightly floral, balsamic, eucalyptus-like scent. It blends well with eucalyptus, rosemary, bay, frankincense, cardamon, pine, cypress, juniper and citrus oils.

PRINCIPAL CONSTITUENTS: Ravintsara is akin to Eucalyptus globulus oil (as both are particularly effective for respiratory infections) due to its high 1,8-cineole content ranging from 45–55 per cent. Other major constituents include monoterpenes (sabinene 15 per cent, alpha-pinene and beta-pinene), sesquiterpenes (beta-carophyllene), monoterpenic alcohols (alpha-terpineol 7 per cent and terpineol) and esters (terpenyl acetate).

NOTE: Ravintsara essential oil contain at least 45 per cent 1,8-cineole compared to approximately 5 per cent found in Ravensara aromatica oil.

SAFETY DATA: Ravintsara oil is generally very well tolerated by the majority of the population, although like eucalyptus, it can cause sensitization in some individuals and should be used in moderation.

Thyme, Common

Thymus vulgaris

FAMILY: Lamiaceae (Labiatae)

SYNONYMS: T. aestivus, T. ilerdensis, T. webbianus, T. valentianus, French thyme, garden thyme, red thyme (oil), white thyme (oil).

GENERAL DESCRIPTION: A perennial evergreen subshrub up to 45 cms high with a woody root and much-branched upright stem. It has small, grey-green, oval, aromatic leaves and pale purple or white flowers.

DISTRIBUTION: Native to Spain and the Mediterranean region; now found throughout Asia Minor, Algeria, Turkey, Tunisia, Israel, the USA, Russia, China and central Europe. The oil is mainly produced in Spain but also in France, Israel, Greece, Morocco, Algeria, Germany and the USA.

OTHER SPECIES: There are numerous varieties of thyme – the common thyme is believed to have derived from the wild thyme or mother-of-thyme (T. serpyllum), which is also used to produce an essential oil called serpolet, similar in effect to the common thyme oil.

Another species used for the production of the so-called red thyme oil is particularly the Spanish sauce thyme (T. zygis), a highly penetrating oil good for cellulitis, sports injuries, etc. (although, like the common thyme, it is a skin irritant). Other species used for essential oil production include lemon thyme (T. citriodorus), a fresh scented oil good for asthma and other respiratory conditions, safe for children. Spanish marjoram (T. Mastichina) is actually botanically classified as a variety of thyme, with which it shares many common properties, despite its common name. Spanish marjoram contains mainly 1,8-cineole (up to 75 per cent) and linalool (up to 20 per cent), so therapeutically has much in common with eucalyptus, being well suited to treating viral and bacterial respiratory infections. HERBAL/FOLK TRADITION: One of the earliest medicinal plants employed throughout the Mediterranean region, well known to both Hippocrates and Dioscorides. It was used by the ancient Egyptians in the embalming process, and by the ancient Greeks to fumigate against infectious illness; the name derives from the Greek thymos meaning ‘to perfume’. It is also a long-established culinary herb, especially used for the preservation of meat.

It has a wide range of uses, though in Western herbal medicine its main areas of application are respiratory problems, digestive complaints and the prevention and treatment of infection. In the British Herbal Pharmacopoeia it is indicated for dyspepsia, chronic gastritis, bronchitis, pertussis, asthma, children’s diarrhoea, laryngitis, tonsillitis and enuresis in children.

EXTRACTION: Essential oil by water or steam distillation from the fresh or partially dried leaves and flowering tops. 1. ‘Red thyme oil’ is the crude distillate. 2. ‘White thyme oil’ is produced by further redistillation or rectification. (An absolute is also produced in France by solvent extraction for perfumery use.)

CHARACTERISTICS: 1. A red, brown or orange liquid with a warm, spicy-herbaceous, powerful odour. 2. A clear, pale yellow liquid with a sweet, green-fresh, milder scent. It blends well with bergamot, lemon, rosemary, melissa, lavender, lavandin, marjoram, Peru balsam, pine, etc.

PRINCIPAL CONSTITUENTS: Thymol and carvacrol (up to 60 per cent), cymene, terpinene, camphene, borneol, linalol; depending on the source it can also contain geraniol, citral and thuyanol, etc.

There are many chemotypes of thyme oil: notably the ‘thymol’ and ‘carvacrol’ types (warming and active); the ‘thuyanol’ type (penetrating and antiviral); and the milder ‘linalol’ or ‘citral’ types (sweet-scented, non-irritant).

SAFETY DATA: Red thyme oil, serpolet (from wild thyme), ‘thymol’ and ‘carvacrol’ type oils all contain quite large amounts of toxic phenols (carvacrol and thymol). They can irritate mucous membranes, cause dermal irritation and may cause sensitization in some individuals. Use in moderation, in low dilution only. They are best avoided during pregnancy.

White thyme is not a ‘complete’ oil and is often adulterated. Lemon thyme and ‘linalol’types are in general less toxic, non-irritant, with less possibility of sensitization – safe for use on the skin and with children.
